
Cardiac surgeon reveals why silent heart attacks are a risk for even young and fit Indians: Causes to prevention tips
Can you have a heart attack and not know it? Silent heart attacks can be particularly dangerous because they often go unnoticed, and the damage may only be discovered later.
In an interview with HT Lifestyle, Dr Niranjan Hiremath, senior consultant and cardiovascular and aortic surgeon at Apollo Indraprastha, New Delhi, said that silent heart attacks are a growing concern among young Indians. Dr Hiremath said, 'It's an alarming thought — someone in their 30s or 40s, apparently healthy and active, suddenly collapses or ends up in an emergency room, diagnosed with a heart attack, which they never saw coming. These are not just rare exceptions anymore. In this digital era, increasingly, cases of 'silent heart attacks' are being reported among youngsters, who show no obvious signs of heart trouble.'
Estimates suggest that 22 percent to 60 percent of all heart attacks are silent, and that such attacks may be more common in women or people with diabetes, according to the Cleveland Clinic. But what exactly is a silent heart attack, and why is it affecting people who appear to be in the prime of their health?
Dr Hiremath said: 'Unlike the dramatic chest-clutching scenes we generally associate with heart attacks, a silent heart attack can occur with little or no noticeable indications. Medically known as a 'silent myocardial infarction' — it damages the heart muscle just like a regular heart attack, but goes overlooked, often until much later.' He added that sometimes, symptoms are so mild or unusual that they are brushed off as fatigue, acidity, gas-related problems or muscle pain.
According to Dr Hiremath, the stereotype of a heart patient being overweight, sedentary, or elderly no longer stands. He said that 'many teens with healthy-looking bodies and active lifestyles are now part of heart disease statistics. 'Stressful work environments, irregular sleep cycles, processed foods, smoking, drinking, and excessive gym routines without proper heart checks are all part of the problem,' Dr Hiremath said.
He added, 'Another major risk factor is genetics. In general, Indians are genetically more prone to heart disease at an earlier age due to smaller coronary arteries and a higher prevalence of diabetes and cholesterol issues. So even if someone looks fit from the outside due to their physique, they may be carrying silent risk factors inside.'
The tricky part about silent heart attacks is the lack of typical symptoms. 'But some subtle signs can still be there — mild chest discomfort, breathlessness during exercise, unexplained fatigue, pain in the jaw, neck or arms, dizziness, or indigestion-like discomfort – which are often ignored or misread as something harmless, especially by the youth,' Dr Hiremath said.
He added, 'It's also common for people to assume that being slim or going to the gym daily automatically protects them from heart related issues. But physical appearance or physique doesn't always reflect what's going on inside the arteries.'
Even if you're a teen and have no symptoms, according to Dr Hiremath, getting an annual health check-up — including ECG (electrocardiogram), cholesterol tests, blood pressure, and if necessary, a stress test — can help detect early warning signs. He said, 'Individual with family history of heart disease, diabetes, or high blood pressure should be extra cautious. Also, fitness should be holistic — combining regular exercise and gym, a balanced diet, adequate sleep, and stress management. Over-exercising without medical clearance or extreme dieting can do more harm than good.'
Dr Hiremath also spoke about how by being aware of the risks and taking proactive steps, people can reduce their likelihood of experiencing a silent heart attack and improve their overall cardiovascular health.
He suggested you start with small, sustainable lifestyle changes: 'Quit smoking, cut down on processed food and sugar and manage your weight. Also, practice yoga, meditation or deep breathing to control stress. Sleep at least 7–8 hours daily. And most importantly, don't ignore what your body is trying to tell you. If something feels off/bad, get it checked.'

A silent heart attack doesn't announce its arrival, but the damage it causes can be lifelong or, worse, fatal. Whether you're a busy professional, a gym enthusiast, or just someone who feels fine, your heart deserves attention. Don't wait for a warning or signs — stay ahead of it,' Dr Hiremath concluded.

Often, foot pain may be brushed aside, but it can signify a range of underlying conditions, both health-related and lifestyle habits that may be harming you on a daily basis. Dr Garima Gupta, Senior Consultant at Nivaan Care, shared with HT Lifestyle how chronic foot pain in women can impact everything from posture and pelvic health to hormonal balance, making it important to prioritise foot pain. Dr Garima Gupta explained, 'Women frequently suffer with foot pain silently, attributing it to 'normal'.Chronic foot pain is often a silent disruptor of everyday routine. Women often experience inconveniences ranging from heels that hurt or stiffness in their soles (especially first thing in the morning or after long hours in the kitchen) to hormonal shifts during periods, pregnancy or menopause; women's feet go through a lot. Foot pain is not really a matter of localisation but how the entire body feels- postures, movements, and even hormones.' Dr Garima Gupta shared a detailed guide with us, covering the reasons and consequences of foot pain: Reasons for foot pain 1. Hormonal fluctuations Hormones like relaxin and estrogen levels rise during menstruation, pregnancy, and in the perimenopause, contributing to extra ligament laxity. The softening affects the pelvis, but also the small ligaments in the feet. This laxity becomes evident in the form of flatfoot or plantar fasciitis, and it alters posture and gait. Solutions: Take care of your foot arches during hormonal changes. Explore a footwear change in the stage of pregnancy or menopause, consider arch supportive footwear and/or customised orthotics. Yoga poses such as Tadasana well as standing on one foot can activate foot musculature, could be useful for women in their 30's and 40's in order to maintain an active arch. Long hours of standing for household work In India, there are factors like long exposure to standing due to housework, ill-fitting, narrow-sole footwear, or walking barefoot on hard marble floors indoors. All these cultural factors contribute to chronically stressful feet. As a result, this impacts pelvic posture, mobility, and spinal alignment. Solutions: Implement deliberate foot breaks: regular foot massaging with a textured foot roller in the kitchen or workstation. If standing for long periods, use silicone heel cups, or other gel inserts. 10 minutes of earthing (daily barefoot walking on grass) can help recalibrate the body's biomechanics and relieve foot tension. 3. PCOS Women with PCOS often have foot pain that is unexplained or tissue recovery, burning feelings, and symptoms that derive from systemic inflammation and insulin resistance. Inflammation restricts activities and worsens metabolic markers, increasing the risk of plantar fasciitis and stiffness in the Achilles' tendons. Solutions: Implement a level of integrated elevation, not only orthopaedic but metabolic, for women with PCOS or metabolic syndrome who experience foot pain. Under supervision, anti-inflammatory diet, magnesium supplementation, and low-impact movement, such as aqua therapy or rebounding can decrease the load experienced on joins and improve insulin sensitivity. Poor posture Chronic foot pain can gradually change posture (i.e.) shoulders come forward, step length shortens, head goes down. All of this can, over time, start to change how a woman presents herself to the world, and subsequently can change her self image and confidence levels. Studies have found a direct relationship between posture and cortisol levels, which can affect mood, weight, and hormone levels. Solutions: Continuing Pilates or even pursuing some form of posture-alignment therapy can help, while also improving mental well-being. 10-minute posture resets like wall angels, or neck lengthening stretches, or lying flat with feet on the wall can make a considerable difference in body language behaviour and mood management for women who have a lot of plates that they are spinning. 2. Poor pelvic floor health When foot pain alters your gait, the change in movement will cause your body to stabilise by engaging muscles (e.g. glutes and pelvic floor) in a dysfunctional manner. This will either lead to pelvic floor tightness (in the case of painful periods), or weakness (in the case of urinary incontinence). Solutions: Work with a physiotherapist to assess foot posture and pelvic alignment under expert guidance. Diaphragmatic breathing and foot-to-core integration exercises (eg: scrunching toes while deep breathing) are a great way to re-integrate pelvic floor activity with posture. Chronic Kidney Disease (CKD), in particular, can advance unnoticed for years. Recognising the signs early can make a significant difference in slowing its progression.' Dr. Mohit Khirbat shared the early warning signs of kidney illnesses that we should not overlook: 1. Constant tiredness or lack of energy: As kidney function reduces, waste products begin to build up in the blood. This may produce a feeling of tiredness, lack of energy, and inability to concentrate. Decreased production of erythropoietin, a hormone produced by the kidneys and which assists in producing red blood cells, can also result in anaemia, adding to tiredness and breathlessness on even slight exertion. Too often, patients tend to write this off as normal fatigue or as an aspect of ageing. 2. Visible changes in urination: Changes in your urination are usually the first signs of kidney trouble. These may be nighttime urination (nocturia), foamy or bubbly urine (protein loss), hematuria (blood in urine), or deeply colored urine. These changes may appear harmless or transient but may be a sign of a developing kidney issue that should be looked into. Foamy or bloody urine is a sign of kidney disease.(Freepik) 3. Swelling in lower limbs or near eyes: Impaired kidney function may result in fluid retention, which manifests as puffy eyes, ankles, or feet. This happens when the kidneys are no longer efficiently removing excess fluids and salt. Though many blame such swelling on eating or standing for long hours, it might indeed be an early sign of kidney issues. 4. Itchy skin or unexplained rashes: Recurring itchiness without an apparent dermatological basis is most likely associated with a waste accumulation and mineral imbalances such as calcium and phosphorus. Nausea, metallic taste, or decreased appetite: As kidney function declines, toxins accumulate in the blood, impacting the digestive tract. These properties make it beneficial in preventing lifestyle diseases like heart problems, type 2 diabetes, obesity, and even certain cancers.' 'Antioxidants in the oil are also helpful to protect a person against cardiovascular diseases, type 2 diabetes, obesity, and cancer, which are also called non-communicable diseases. Oxidative stress or the free radical generation in the body is pretty much high in these cases. In these diseases, and also in people with other health problems, adding the antioxidants or the immune nutrient in the body helps to fight against the oxidative stress or the free radicals caused by these diseases or the medicines which are taken to treat these diseases,' Swatee Sandhan explained. 'While it offers health benefits, olive oil must be used mindfully to avoid weight gain and preserve its nutrients,' said the dietician. She further shared the cons of using too much olive oil. Just one teaspoon has 40 calories, so it adds up fast. It is also not ideal for deep frying, as its smoke point is lower than some Indian cooking oils. The smoking point of olive oil is 190°C, which makes it suitable for using in cooking, sautéing, roasting. Overheating olive oil can destroy its antioxidants and make it harmful. People with certain allergies or those taking blood thinners should be cautious and consult a doctor before using olive oil regularly.
