How screen use can be a vicious cycle for some children and adolescents

To cope with behavioral problems linked to excessive screen use, such as aggression or anxiety, some children take refuge... in screens. While this vicious circle aspect of the screen phenomenon is not widely discussed, it has been highlighted by a comprehensive Australian study published this week.
It has long been known that screens are harmful to children. Excessive exposure (several hours a day) has been linked to cognitive development delays, as well as learning, language, and memory disorders. And in adolescents, excessive screen use may cause manic symptoms, such as mood disorders and sleep disturbances. But a new meta-analysis highlights a lesser-known problem.
The study reports that excessive screen use can exacerbate socioemotional problems (anxiety, depression, aggression, hyperactivity, etc) in children... in turn driving them to screens to cope with these issues. This vicious circle mainly affects children aged 6 to 10 and seems to be more prevalent in boys than in girls.
Conducted by psychology professors at the University of Queensland (Australia), the research in question examined around 100 studies involving more than 292,000 children from around the world. The findings are published in the journal Psychological Bulletin .
Most of the studies included in this research were conducted in the United States (41 studies), Canada (13), Australia (11), Germany and the Netherlands (7 each).
The effects of video games
The researchers found that video games are associated with higher risks of developing behavioral disorders compared to other screen-based content, including those intended for educational or recreational purposes.
"As a kid I loved video games – they give you a challenge and powerful feedback when you're doing a good job. For this reason, games are particularly tempting for children having problems and especially hard to get children away from," says study senior author, associate professor Michael Noetel from UQ's School of Psychology, quoted in a news release.
While it's true that some video games have an educational dimension (some of them have even been recognised as a vehicle for mental well-being among teenagers), excessive use can be counterproductive.
"Dr Noetel said parents and policymakers have long debated whether screens cause problems, or whether children with problems were simply drawn to screens," the news release notes. "Our review shows it's both," he adds.
"If we only address one side of the equation – cutting down on screen time but overlooking anxiety or aggression that leads kids to screens – we risk leaving children stuck in a cycle."
However, the researcher tempers this by pointing out that the emotional and behavioral disorders mentioned can also be explained by other factors, such as exposure to alcohol and/or drugs.
Follow the 3-6-9-12 rule
In France, some health professionals now recommend keeping children under six away from screens entirely, which is three years longer than the current official recommendations (ie, no screens before three).
To help parents limit their children's screen time, French pediatrician Serge Tisseron has invented the 3-6-9-12 rule, designed according to children's age and stage of cognitive development.
The rules to remember are as follows: no screens before the age of 3, no personal consoles before the age of 6, no Internet before the age of 9, and no Internet without supervision before the age of 12.
Other initiatives have recently emerged to encourage children and teenagers to "detox' from digital devices, such as the challenge of spending 10 days without using any screens at all, in favor of activities that promote relaxation and creativity. – AFP Relaxnews

When it comes to advanced cancer or Alzheimer's disease, over half of doctors would consider assisted dying for themselves, but preferences seem to vary according to their jurisdiction's legislation on euthanasia, reveal the results of an international survey, published online in the Journal of Medical Ethics . And most say they would prefer symptom relief rather than life sustaining treatment for their own end of life care, indicate the responses. Previously published research suggests that doctors' views on their own end of life care inform their clinical practice, and that their perceptions of their patients' treatment wishes are influenced by their own preferences, note the researchers. But most of the studies on physicians' preferences for end of life practices are outdated and/or narrow in focus, added to which little is known about whether doctors would consider assisted dying for themselves, and whether this might be influenced by national or state legislation on the practice, point out the researchers. Survey in five countries To shed more light on these issues, the researchers surveyed doctors in eight jurisdictions with differing laws and attitudes to assisted dying: Belgium; Italy; Canada; the states of Oregon, Wisconsin, and Georgia in the United States; and the states of Victoria and Queensland in Australia. Physician-assisted suicide law entered the statute book in Oregon in 1997, while Death with Dignity legislation has been introduced in Wisconsin numerous times over the past 20 years but remains illegal. It is also illegal in Georgia which is one of the most religious states in the US. In Canada, both physician-assisted suicide and euthanasia have been permitted since 2016. In Belgium, assisted dying has been legal since 2002, but remains illegal in Italy, one of the most religious countries in Europe. The Australian state of Victoria implemented assisted dying legislation in June 2019. In Queensland, assisted dying legislation was passed in 2021, but had not yet come into force when the data for this study were collected (May 2022–February 2023). Two hypothetical situations were included to probe doctors' views on end of life care: advanced cancer and Alzheimer's disease. Respondents were asked the extent to which they would consider various end of life practices for themselves. These included cardiopulmonary resuscitation (CPR), mechanical ventilation, tube feeding, intensified alleviation of symptoms, palliative sedation, the use of available drugs to end life, physician-assisted suicide, and euthanasia. Responses were sought from family doctors (GPs), palliative care doctors, and other medical specialists highly likely to treat patients at the end of their life, such as cardiologists, emergency medicine doctors, oncologists, neurologists, and intensive care specialists. What's a good option? Of the 1,408 survey responses received, 1,157 were included in the final analysis. These showed that doctors rarely considered life sustaining practices a (very) good option in cancer and Alzheimer's, respectively: CPR 0.5% and 0.2%; mechanical ventilation 0.8% and 0.3%; tube feeding 3.5% and 3.8%. Most (94% and 91%, respectively) considered intensifying symptom relief a good or very good option, while 59% and 50%, respectively, considered palliative sedation a good or very good option. Respondents who considered palliative sedation for Alzheimer's disease as a good or very good option ranged from just over 39% in Georgia to just over 66% in Italy. About half of respondents considered euthanasia a (very) good option: just over 54% and 51.5%, respectively, for cancer and Alzheimer's disease. The proportion of those considering euthanasia a (very) good option ranged from 38% in Italy to 81% in Belgium (cancer scenario), and almost 37.5% in Georgia, to almost 67.5% in Belgium (Alzheimer's disease scenario). Around one in three (33.5%) respondents said they would consider drugs at their disposal to end their own life (cancer scenario). While sex, age and ethnicity didn't seem to influence doctors' preferences for end of life practices, prevailing legislation in their jurisdiction did. Doctors working in a jurisdiction with a legal option for both euthanasia and physician-assisted suicide were three times as likely to consider euthanasia a (very) good option for cancer and almost twice as likely to consider it a (very) good option for Alzheimer's disease. 'This may be because these physicians are more familiar and comfortable with the practices and have observed positive clinical outcomes. 'It also suggests that macro-level factors heavily impact personal attitudes and preferences, and physicians are likely influenced by what is considered 'normal' practice in their own jurisdiction,' say the researchers. Religious views GPs and other specialists were less likely to consider palliative sedation a good or very good option than palliative care doctors, and they were more likely to consider euthanasia, physician-assisted suicide, and the use of available medication to end their own life a (very) good option. And doctors who weren't religious were more likely to consider physician-assisted suicide or euthanasia a preferable option than those with a strongly held faith: physician-assisted suicide 65% vs 38%; euthanasia 72% vs 40%. Due to the study design and nature of surveys the results can't be considered fully representative, and those doctors with a particular interest in the subject may have been more likely to take part, acknowledge the researchers. While the overall recruitment of respondents was satisfactory in all jurisdictions, GPs were under-represented among the Canadian respondents. But note the researchers: 'Our findings show that across all jurisdictions physicians largely prefer intensified alleviation of symptoms and to avoid life-sustaining techniques like CPR, mechanical ventilation, and tube feeding. 'This finding may also relate to the moral distress some physicians feel about the routine continuation of treatment for their patients at the end of life. 'These findings warrant reflection on current clinical practice since life-prolonging treatment is still widely used for patients,yet is not preferred by physicians for themselves.'

(Facebook pic) PETALING JAYA: The High Court in Melaka has ordered the Melaka United Soccer Association (Musa) to pay nearly RM700,000 following the death of an 18-year-old Australian player who was struck by lightning nine years ago. Stefan Petrovski, an 18-year-old goalkeeper with Melaka United, was hit by lightning while training at the Hang Jebat Stadium on April 3, 2016. He died nearly a month later, on May 1, from brain damage caused by a lack of oxygen. Advertisement His father, Marco Petrovski, sued Musa and 15 of its officials for negligence and failing to provide a safe environment for his son. The family claimed that no automated external defibrillators (AED) were available during training, no doctor was present at the session, and training was not stopped despite rain increasing the risk of lightning. They also said Musa failed to take out a personal accident insurance policy as required by the player's contract. Justice Radzi Abdul Hamid ruled that Musa was negligent and had a duty of care to protect Petrovski as a professional player. The court awarded AU$192,000 (RM528,000) for loss of dependency, RM120,000 for failure to insure the player, and RM50,000 in legal costs. The judge rejected Musa's defence that a panel doctor did not have to be present at training sessions, stating that Football Association of Malaysia (FAM) rules required at least one doctor to be available during both matches and training. 'The defendants contended that under FAM regulations, a doctor does not need to be at the training session,' said Radzi. 'This court disagreed with the defendants' contention… It makes no sense to appoint the doctor who is absent and must be called to the location in the event of a medical emergency. 'The defendants also failed to adequately train their staff to use AED, as well as to respond to cardiac related emergencies, which contributed to Petrovski's death.' TOKYO: Sweltering temperatures prompted heatstroke alerts in multiple Japanese regions on Wednesday, with dozens of people seeking emergency medical care in the capital Tokyo. The hot weather was headline news in the country, which last year experienced its joint warmest summer ever as climate change fuelled extreme heatwaves around the globe. Record temperatures were logged in 14 cities for June, the Japan Meteorological Agency said, while in central Tokyo the mercury hit 34.4 degrees Celsius (94 Fahrenheit). Doctors treated at least 57 people for heat-related malaise in the capital on Wednesday, adding to the 169 people seen on Tuesday. At least three heat-related deaths were reported in other parts of the country this week. Some Tokyo residents wore heat-repellent clothing to beat the high temperatures, like Junko Kobayashi, 73, who showed AFP her cooling scarf. "I soak it in water and then wrap it around my neck. It feels refreshing. And I use this umbrella too. It blocks the light and heat so it feels cooler," she said. Other elderly residents said they were trying to take it easy so as not to risk heatstroke, while 80-year-old Naoki Ito said he was making sure to regularly drink water. "I don't need to take a big gulp, just a small sip here and there. It's important to remember that," Ito said. Every summer, Japanese officials urge the public, especially elderly people, to seek shelter in air-conditioned rooms to avoid heatstroke. Senior citizens made up more than 80 percent of heat-related deaths in the past five years. Japan is also experiencing a record influx of tourists, with foreign visitors up 21 percent year-on-year in May. "It's been pretty stinking," said 31-year-old Australian tourist Jack Budd, who was trying to find shade whenever possible with his travel partner.
