National vaccine registry needed amid measles resurgence, Canada's outgoing top doctor says

As Dr. Theresa Tam retires as Canada's top doctor, she's calling for a national vaccine registry.
Tam says the COVID-19 pandemic, which began in early 2020 and killed at least 60,000 Canadians, showed how badly one is needed to track vaccines and protect vulnerable communities.
A national vaccine registry, she says, could help prevent and manage crises like the resurgence of measles that the country now faces.
Tam says she's in favour of a "nationally interoperable network of vaccine registries" that connects all of the provincial and territorial health systems and helps identify pockets of the population where there is poor vaccine coverage.
While the majority of measles cases so far have occurred in communities that are historically under-vaccinated, she told Dr. Brian Goldman, host of CBC's White Coat, Black Art, "we don't actually know exactly where the situation with vaccine coverage lies."
Tam spoke to Goldman from Ottawa for a feature interview reflecting back on her career as the country's chief public health officer.
The pandemic proved that the technology for a registry is there, she says, given that almost all provinces and territories made COVID-19 vaccine records available electronically during that time.
Although it was the pandemic that made her both a household name — and a target for hate — Tam came to the role of top doctor three years earlier, in June 2017, after occupying a series of other leadership positions within the Public Health Agency of Canada (PHAC), including deputy chief. She has also served as an expert on a number of World Health Organization committees.

Leading Canada's public health response to COVID-19 was as much a personal challenge as it was a professional one, given Tam was also on the receiving end of racist and sexist vitriol during that time.
"What I try to do, certainly at the time and even now, is just to focus on the job that I'm trying to deliver," she said.
Those attacks were even harder on her staff — those monitoring the channels where the messages would come in, and who tried, as much as possible, to shield her from the worst of it, Tam says.
"And one member of my staff used to also read me the incredibly lovely cards and messages that the public sent to encourage me to carry on. It was like the antidote to the other messages as well. So that really helped."
One of Tam's provincial counterparts, Dr. Robert Strang, chief medical officer of health in Nova Scotia, says he has "huge respect for Dr. Tam."
"Certainly working with her during COVID, we were all kind of in this kind of team together. Having her leading us as chief medical officers was a real pleasure and a privilege."
Getting cross-Canada support for registry
The challenge with getting a network of vaccine registries operating to help with measles and other infectious disease crises in future, Tam says, lies in getting all of the jurisdictions to sign on and co-operate to make their systems speak to one another.
"There's in fact quite a lot of work on the way right now to develop those agreements with the provinces, as well as the more technical aspects of this," she said.
A national registry, she says, will make things "so much easier" for public health departments, doctors, patients and parents.
"That's the most important thing, that you will know whether your kids got the vaccine and that your health provider can have that information as well."
Strang says there's agreement on the principle, but "the devil is always in the details."
There are a number of groups, including PHAC, working to get the federation to do a better job of sharing health data, he says.
"There's certainly support and agreement amongst the chief medical officers and public health leaders of this being a priority," Strang said.
But Dr. Iris Gorfinkel, a Toronto family physician and researcher, says she suspects political considerations are behind some of the provinces "literally hoarding their medical data."
"When politics stands in the way of science moving forward or health being prioritized, it becomes a serious, potentially life-threatening problem," said Gorfinkel, who is also the founder of PrimeHealth Clinical Research.
Some provincial governments may be afraid voters will dislike sharing their health data, she says.
But that shouldn't be a factor, given how easy it is to remove identifying details from the information so no one apart from an individual and their health-care provider knows their vaccine status, Gorfinkel says.
We're already paying a heavy price — in lives and dollars — for not having a national registry, she says.
"First, there's a tremendous waste of vaccines," Gorfinkel said. We saw that during COVID-19, she says, when Canada over-bought vaccines and struggled to administer them before they expired, eventually drawing international criticism for doing so while other parts of the world went without.
In the case of the measles outbreaks, she says, "if we don't know where those are happening and we can't ship vaccines to where they're most needed, then what that means is we're not going to make the best use of the vaccines we have."
And, of course, people and health systems pay when hospitals fill with people who have become ill from vaccine-preventable illnesses, Gorfinkel says.
Looking abroad
Other countries have made this work, among them Sweden, France, Finland, Germany, the Netherlands and Spain.
In Norway, for example, a national registry that began in 1995 and at first tracked only routine childhood vaccinations, was expanded in 2011 to require mandatory reporting for all vaccines and age groups.
Gorfinkel says some of these countries better embrace the sentiment that "what happens to my neighbour does affect me."
Public health expert Amir Attaran, a vocal critic of Tam and PHAC during the pandemic, says far more could have been done at the federal level to get a national registry in place in this country, too.

A professor in the faculties of law and school of epidemiology and public health at the University of Ottawa, Attaran says infectious disease experts like Tam have known since the SARS outbreak in 2003 that the country was woefully unprepared to properly track and respond to an outbreak due to poor data sharing.
"And because of the experience of SARS in Canada, we should have been very switched on about this," said Attaran, who holds both a law degree and a PhD in biology.
He says Canada's Statistics Act empowers the federal government to "require the provinces to cough up statistics," and that's exactly what public health data is.
Canada's public health culture is self-defeating, Attaran says, because it recognizes the necessity of accurate and timely data but proceeds "under this false assumption that you can't get those data unless everybody agrees and everybody's happy with it. And then you fail to put in the political energy to bring about that consensus by agreement or coercion."
Masking controversy
While the pandemic highlighted the need for a national vaccine registry to manage outbreaks better, it also exposed challenges in public health communication.
One of the most debated issues was masking.
Attaran was among those who said Tam bungled the advice around masking in the earlier stages of the pandemic, for example.
"Early in the pandemic, she told people, 'No, you don't need to mask,'" he said. Yet by then the virus had been identified as SARS-CoV-2, and we'd already learned from the 2003 SARS crisis that it required masking, he says.
But in Nova Scotia, Strang says, the information scientists were working with early on suggested the virus behaved like an influenza, which spread through droplets, not aerosols. "We don't recommend widespread masking, necessarily, for everybody for influenza."
Tam says public health leaders "need to do a better job in explaining to people how the scientific information are coming through, how we analyze it and how we turn them into guidance."

If you haven't subscribed yet, you can do that by clicking here. Anorexia is a life-threatening eating disorder that can manifest as an intense preoccupation on weight loss. It's classified as a mental illness and normally treated with talk therapy, known as family-based therapy, but that's only effective for about half of the mostly women and girls who have it. Those who don't improve may go to hospital and get locked into a relentless cycle of gaining weight and recovering from malnourishment, followed by weight loss and damage to organs, including changes to the brain from starvation. Now, Canadian researchers are testing a new approach, tapping into the growing understanding of the gut-brain connection. They're going to try treating teens with fecal transplants, to change the bacteria in their gut. "We know that once the symptoms set in and the brain changes, it's really hard to change the course, so if we can intervene early in the adolescent years, we have the best chance," said Dr. Jennifer Couturier, a child psychiatrist and associate professor of psychiatry at McMaster University in Hamilton, Ont. In fecal microbial transplants, a small sample of stool from a healthy person is purified to concentrate the beneficial bacteria minus the waste. The microbiome is then put in a capsule, which the recipient takes for therapeutic purposes. Couturier and her team have Health Canada approval to run a randomized clinical trial in an initial group of 20 females aged 12 to 17 diagnosed with anorexia. They will either take oral capsules along with standard family-based treatment to stabilize nutrition and avoid the harmful effects of starvation or family-based treatment alone. McMaster's own stool bank Fecal transplants have been tested in a small handful of adults with anorexia, including one who restored her weight. In this trial, the McMaster researchers will use the university's own stool bank of donations. Dr. Nikhil Pai, a gastroenterologist at McMaster and a co-investigator on the trial, credited the university's contributions for funding the research and starting the pediatric stool bank. "This is not affiliated with any company," Pai said. "We developed this entirely in-house using carefully screened pediatric donors." Pai said the screening checks for infectious diseases, similar to how blood donations are assessed. Staff at the stool bank then prepare the capsules, which can't be mass produced. WATCH | Exploring the power of poop: There are a few reasons Pai and some other doctors are optimistic participants will benefit. One comes from animal studies. Researchers transplanted gut microbes from humans with anorexia to normal mice models, and found the mice reduced their food intake as if anorexic. Then, when gut microbes from healthy humans were given, the effect reversed. Secondly, young guts are subject to change. Pai said the gut microbiome is malleable in children and teens. "It actually is a very different landscape in adolescence, where if you can make a change, you not only may see some improvement in terms of outcomes for anorexia nervosa in the short term, but … the sustained response," Pai said. Butterflies in the stomach With every meal, trillions of gut bacteria turn breakfast, lunch and dinner into molecules called metabolites that affect the brain. Pai, who also works at Children's Hospital of Philadelphia, said large population-based studies show a few types of bacteria can have an outsized effect on the brain through the gut-brain axis, a connection between the two. "When we feel a bit nervous, sometimes we feel butterflies in our stomach," as a symptom of the nervousness, Pai said. "It's no surprise that we often have these intestinal symptoms that come along with the psychological anxieties." Researchers will follow participants in the clinical trial for eight weeks while they receive the capsules and then for four weeks after that. Pai and Couturier say one aspect they'll record is whether participants are willing to take the fecal transplant — or repulsed by it. Then they'll measure how the patient is doing — by tracking their weight, and mood — and how the microbiome changes based on saliva, urine and stool samples. Gut commands the brain? Scientists are learning more about how gut microbes influence brain function. Various studies have shown people with some psychological problems, including anorexia nervosa, having abnormal intestinal microbes, said Dr. Howard Steiger, former director of McGill's eating disorder program. "We always like to think of the brain as being the command centre for the body," Steiger said. "But you know, a lot of recent findings suggest the gut also controls your brain." In people with anorexia, Steiger said, the thought is that their eating behaviour messes up the way gut microbes influence functions in the brain and body. Given that, transplanting purified fecal samples from healthy individuals could help those with anorexia, said the professor emeritus of psychiatry, who continues to treat people with eating disorders "It's not like total voodoo to think that [fecal transplant] would be a potential adjunct in treatment or maybe even a treatment in itself," Steiger said. Some clinicians now refer to anorexia as a "metabo-psychiatric problem," meaning it is associated with vulnerability both in the brain and the body. "I think that's important because it reduces shame," Steiger said. "People don't develop anorexia because of moral weakness. They carry a real, physical, heritable susceptibility." That means anorexia gets triggered by a combination of nature, or genetics, and nurture, such as life events. Making meal time comfortable Anita Federici, a clinical psychologist north of Toronto, treats adolescents with anorexia using family-based therapy. Federici said initially, the parents or caregivers are empowered to learn how to nourish their child by taking over all meal and snack preparation and supervising intake and weight checks. Then the child might start eating lunch at school unsupervised. In the last phase, the youth regains independence over eating and exercise. About half of younger people with anorexia nervosa who receive family-based treatment for it do really well, Federici said. Change pace of meals? But anorexia rarely "flies solo," Federici said, noting many people also have co-occurring suicidal thoughts or experience self injury, substance use, trauma or PTSD, as well as neurodiversity like autism. All can complicate the picture. Federici said the standard approach of medicalizing treatment for anorexia focuses on gaining weight. Treatment that doesn't take into account the metabolic underpinnings of anorexia can miss the mark, Federici says. "There was research not long ago that demonstrated that there were more serotonin receptors in your gut than there were in your brain," she said, referring to a neurotransmitter with roles including influencing mood. Taking the metabolic basis of anorexia into consideration could mean clinicians may need to change how they feed patients, both in terms of the types of food and the pacing of meals, Federici said. In people with anorexia, the body is dysregulated so they feel soothed when they eat less. While bystanders may think the patient isn't motivated to change, Federici said in reality they're quite motivated. "What I have observed over the years is that these people desperately want to get better. They desperately want a different life, but they need the treatments to be different."

4 people are missing after helicopter crashes on Quebec's North Shore

Four people are missing after an Airmedic helicopter crashed near Natashquan, a community on Quebec's North Shore, around 10:30 p.m. Friday. The helicopter, carrying four crew members and one passenger, was on a rescue operation in the area, according to Raphaele Bourgault, a spokesperson for the company which specializes in medical air transport. One person was rescued and transported to hospital. Their injuries are not life-threatening, according to provincial police, the Sûreté du Québec (SQ). Ground and aquatic search teams are on site to try and find the four missing individuals. "Airmedic personnel are working closely with authorities and first responders to manage the situation and provide assistance," the company said in a statement. The SQ and Canada's Transportation Safety Board are investigating the cause of the crash.

Sask. NDP and ALS society calling on province to investigate Moose Jaw health centre

Social Sharing Saskatchewan's Opposition NDP and the province's ALS society are calling on Minister of Health Jeremy Cockrill to launch an investigation into the Dr. Goodenowe Restorative Health Center in Moose Jaw. Last week, the CBC reported that Dr. Dayan Goodenowe, who runs the centre, tells patients with ALS that his supplements can stop and reverse the progress of the disease. On his YouTube account, he says, "stopping the progression of ALS actually isn't that hard." He backs up his claim by pointing to some patients who say they feel better on his supplements. He also told CBC he's working on studies to verify their effectiveness. Goodenowe charges ALS clients $75,000 US for his three-month live-in treatment program. In CBC's story, some patients who took part said it did nothing to improve their condition. The NDP and the ALS Society say there's no published scientific evidence to back up Goodenowe's claims. They say all of the science indicates ALS is a death sentence. They said businesses should not be allowed to prey on vulnerable people. "We're talking about people with an incurable disease, who are desperate for hope, being misled with promises that are not backed in any scientific evidence. This is false hope being sold for profit," NDP rural and remote health critic Jared Clarke said at a news conference Friday. "We cannot allow unregulated facilities to target vulnerable people with misleading claims without any oversight or accountability. So today we are urging Minister Cockrill to act." Denis Simard, executive director of the ALS Society of Saskatchewan, echoed that call at the news conference, saying "we would invite the ministry to do a special investigation." Simard made a similar request months ago. In a Feb. 12 letter, he asked Cockrill to look into Goodenowe's business and "take appropriate action to prevent the dissemination of false medical information that could exploit those facing ALS." 'It is a private business' Cockrill refused CBC's request for an interview. In a written statement, he indicated there's nothing the provincial government can do because Goodenowe's centre, "has no ties to the Saskatchewan health care system." In interviews with CBC, Goodenowe regularly emphasized that he is not a medical doctor and his facility does not offer medical treatments or services. He has a Ph.D in Medical Sciences with an emphasis in psychiatry from the University of Alberta. "It is a private business, not a provincially regulated health centre or affiliate," Cockrill wrote. "The individual operating it is not a licensed medical doctor, so he does not fall under any established health regulatory body." Clarke said that if Cockrill is accurately describing the situation, that's an even stronger reason for him to do something. "To me that's why an investigation is important, to be able to assess whether there is a gap here [and] whether we need additional legislation," Clarke said. Simard agreed. "OK, we don't have an actual place in the law that allows us to investigate this. Let's create one," he said. "Let's find a way that's going to allow us to protect the most vulnerable in our province." In its research, CBC was directed to section 80 of Saskatchewan's Medical Profession Act, 1981, which seems to indicate that it is against the law in the province for someone who is not a registered medical professional to offer medical services. It says it's an offence in Saskatchewan if someone, "for hire, gain or hope of reward (i) engages in, professes to engage in or advertises to give advice in any aspect of practice; or (ii) furnishes any medicine or treats any disease or ailment by medicine, drugs or any form of treatment, influence or appliance." If found guilty of this offence, "and liable on summary conviction," the offender would be subject to a fine of up to $5,000. CBC asked the minister if this law might apply in this case. He didn't respond to the question. Goodenowe expanding operations In his written statement, the minister added that "we are not aware of any Saskatchewan residents living with ALS who have accessed Mr. Goodenowe's services." He didn't explain why he made that point. Simard noted that in CBC's story, the ALS patients were all from the United States. "These are all people that are being harmed by this process and I think we have a right to protect them," Simard said. He said this question is not going to go away for the provincial government, because Goodenowe is planning to expand his operations in Moose Jaw. In April, Goodenowe announced the Moose Jaw Vitality Project, which his website describes as "a $100 million community health initiative providing free access to cutting-edge health services and monitoring technologies for all residents of Moose Jaw, Saskatchewan." The project is planned to involve five facilities, including the restorative health centre, a cafe and a facility that will manufacture Goodenowe's supplements, which he has manufactured in the United States until now. Simard said Moose Jaw city council should be paying attention to this as well. "Let's make sure that this person has no approvals moving forward, so that until this is fully investigated, until we can fully validate what is being done by this individual, nothing should happen at any other level," he said. For his part, Simard said he has warned Saskatchewan's 73 ALS patients that they should have nothing to do with Goodenowe or his facility.
