First-of-Its-Kind Study Identifies How The Brain Could Thwart Alzheimer's

Training the brain's immune system to recognize and clear toxic material is rapidly emerging as an promising way to put the brakes on Alzheimer's disease. Unfortunately researchers haven't been clear on how this method of protection operates on a cellular level.
An international team of researchers analyzed brain samples taken from people who had died with Alzheimer's, some of whom had also received approved Alzheimer's immunotherapy treatments. The therapies encourage cleaning cells called microglia to attack the clumps of amyloid-beta proteins that are thought to be involved in neurodegeneration.
Microglia responses to amyloid beta can lead to inflammation, which in turn risks damage to brain tissues. The researchers wanted to know why immunotherapy turned microglia into ruthless cleaning machines in some cases but not others.
"Our study is the first to identify the mechanisms in microglia, the brain's immune cells, that help limit the spread of amyloid in certain brain regions following treatment with amyloid-targeting drugs," says neuroscientist David Gate, from the Northwestern University Feinberg School of Medicine.
The team observed microglia not only remove amyloid beta protein clumps but also contain the inflammatory response necessary for this removal, helping the brain recover.
These insights were obtained in part through a technique called spatial transcriptomics, which can highlight activity of targeted genes. The genes linked to supercharged microglia included APOE and TREM2, both of which have been identified as being related to Alzheimer's disease in earlier studies.
While it's early days for this field of research, the findings from the study could potentially lead to new ways to put a stop to amyloid beta build-up and the development of Alzheimer's symptoms.
"A long-standing question in the field of Alzheimer's therapeutics is if we coax these immune cells into removing the amyloid, are they just always going to be in that amyloid-removal mode?" says Gate.
"The answer we found is no, they can remove the amyloid and then go back to being good and appear to actually help the brain heal."
Putting the body's own defense force through a better training scheme is always going to be preferable to introducing additional chemicals into the brain, and the researchers hope that the disease brakes can be applied before Alzheimer's gets a grip.
And while the immunotherapies we mentioned at the top are approved by regulators and can help battle Alzheimer's disease, there's a lot of room for improvement in terms of increasing their success rate and reducing the risk of associated side effects.
"I think everyone agrees that, although these drugs are becoming more effective, they ultimately do not cure Alzheimer's disease patients," says Gate.
"These drugs stimulate the immune cells of the brain to remove amyloid beta, but we believe that the data in our publication can be utilized to make these drugs work even better."
The research has been published in Nature Medicine.

Drugs in the same family as Ozempic and Wegovy are known for treating diabetes and helping with weight loss, but a small, early-stage study suggested they might also ease migraines -- even when there's no weight loss. The benefit appears to come from lowering pressure in the brain, Dr. Simone Braca, a neurologist at the University of Naples Federico II and lead author of the study, explained to ABC News. 'This study is very interesting in that the GLP 1s are hypothesized to lower brain pressure, which can then lower your chance of getting a headache or a migraine,' Braca said. The small, 12-week study tracked 26 adults with obesity who had chronic or frequent migraines. Published in Headache -- the official journal of the American Headache Society -- and presented at this week's European Academy of Neurology meeting in Finland, it tested liraglutide, a type of GLP-1 drug commonly used for diabetes and weight loss. After taking a daily 1.8 mg dose of liraglutide for three months -- the amount typically used to treat diabetes -- their average number of headache days per month dropped from 20 to about nine. Participants also reported less disability from migraines, with scores on a standard headache impact scale cut by more than half. Although some participants lost a small amount of weight, Braca said the few lost pounds were not meaningful enough to explain the improvement in migraines. Instead, Braca pointed to pressure from cerebrospinal fluid -- the liquid that surrounds and cushions the brain and spine. He said he believes that even slight buildups of this fluid can press on nearby veins and nerves in the brain, potentially triggering migraines. 'An increased pressure of the spinal fluid in the brain may be one of the mechanisms underlying migraine,' Braca said. 'And if we target this mechanism, this preliminary evidence suggests that it may be helpful for migraine.' Nearly half of patients reported at least a 50% reduction in headache days, according to the Headache paper. About 40% experienced mild side effects like nausea or constipation. None stopped taking the medication. With such promising results, Braca and his research team, led by Dr. Roberto De Simone, are already planning larger trials. Future studies will measure brain pressure more directly and explore whether other GLP 1 drugs might also offer the same relief but with fewer side effects. 'There are still a substantial portion of migraine patients that face an unmet need and that live with its burden,' he said. 'New drugs that could target other pathways, I think that could be reassuring to those patients and give them hope.' The study adds to growing evidence that GLP-1 drugs may have benefits beyond diabetes and weight loss. Researchers are already studying these medications for a range of other conditions, including reducing the risk of heart disease and stroke, easing symptoms of addiction and treating Alzheimer's disease.

Microplastics may be tiny in size, but a recent study shows they have a huge impact on our crops. Worrying new research has been highlighted by detailing the profound impact that microplastics have on soil and the crops that grow in contaminated areas. The study, originally published in the Environmental Sciences Europe journal, found that "agricultural soils now hold around 23 times more microplastics than oceans." The review stated that microplastics and nanoplastics have been discovered in lettuce, wheat, and carrot crops, with the lead researcher, Joseph Boctor, saying, "These microplastics are turning food-producing land into a plastic sink." Microplastics are flakes of plastic less than 5 millimeters in length. Plastics that can't biodegrade break down into smaller and smaller pieces, which are difficult to get rid of. They are incredibly pervasive, entering oceans, soil, animals, and human bodies. Animals and humans can consume the particles that end up in water, soil, and food sources, and microplastics may even be inhaled. Unfortunately, the presence of microplastics has been linked to serious health issues. In hopes of preventing the creation of microplastics in the future, Boctor and his colleagues at the Bioplastics Innovation Hub are in the process of creating plastic that decomposes in both soil and water. More eco-friendly kitchen utensils and containers, made of materials such as wood, glass, or ceramic, can prevent the release of microplastics in the dishwasher.
