Trump budget's caps on grad school loans could worsen doctor shortage

The price of a four-year program in osteopathic medicine is $297,881 at a public school, on average, and $371,403 at a private school, according to the American Association of Colleges of Osteopathic Medicine. The average indebtedness of their graduates is $259,196.

The proposed loan caps 'will either push students and families into the private loan market, where they take on more risk and have less consumer protection, or simply push people out of higher education altogether,' said Aissa Canchola Bañez, policy director at the Student Borrower Protection Center, a nonprofit advocacy group.
Private student loans are also not eligible for Public Service Loan Forgiveness programs, which many students rely on to manage their debt. Students from low-income families may have difficulty qualifying for private loans.

In a letter to congressional leaders, the American Medical Association asked lawmakers to carve out exceptions in the law for medical education, saying that the current bill would deter good candidates from applying to medical school, discourage physicians from working in underserved areas, and make medical school unaffordable for all but the very wealthy. Critics said it could also drive more doctors away from lower-paying primary care fields, an area of acute need, and into more lucrative specialties.
Conservatives have argued for decades that the availability of federal student loans allowed tuition costs to balloon — a proposition rejected by the Association of American Medical Colleges, which instead blames the rising cost of living. Studies examining the relationship between loans and tuition have varied in their conclusions.
Sara Robertson, press secretary for Republicans on the House Education and Workforce Committee, said the proposed loan limits 'will drive down the cost of medical school and thus reduce the need for students to borrow in the first place.'
She said private lenders would offer students lower interest rates than the government's Grad PLUS program would. The private market, though, has shrunk markedly since the Great Recession, according to Lesley Turner, an associate professor of public policy at the University of Chicago and an author of an unpublished paper that linked loans to rising tuition.
In an email, Turner said it wasn't clear that 'the same level of nonfederal funding would be available today.' Because of inflation, she noted, the $150,000 limit for medical students is 'substantially reducing the amount these students can borrow, even compared to the status quo before the Grad PLUS program.' The program was started in 2006.

Robertson said schools could also help close the gap. 'Nothing in the bill prevents colleges from providing additional financial aid to low-income students pursuing medical school,' she said.
That is not an option for schools of osteopathic medicine, most of which are private and not attached to universities with foundations, and which currently enroll almost one-third of the nation's future doctors, said David Bergman, senior vice president of government relations and health affairs for the American Association of Colleges of Osteopathic Medicine. And many academic medical centers, which have lost millions of dollars in research grants abruptly pulled by the Trump administration, are facing severe financial strain.
A vast majority of medical students rely on loans, not just those from low-income backgrounds, Bergman noted.
Ending federal involvement in administering and subsidizing student loans was one of the goals laid out in Project 2025, the Heritage Foundation's conservative blueprint for overhauling the government. It argued that leaving student loans to private lenders and ending federal loan forgiveness programs would 'allow for market prices and signals to influence educational borrowing.'
Trump's policy bill would allow medical residents to defer not only their loan payments but also the interest on those payments, a provision that many medical professionals have supported. But the legislation would prohibit residents from counting those low-paid years of training as public service, limiting their eligibility for a popular loan forgiveness program that encourages young doctors to work in underserved areas.
As a result, 'access to much-needed medical care for patients in rural and underserved communities will be diminished,' the American Medical Association's CEO, Dr. James Madara, wrote in the letter to House Speaker Mike Johnson, Republican of Louisiana, and House Democratic leader Representative Hakeem Jeffries of New York.

The changes will disproportionately affect low-income students with backgrounds that are underrepresented in medicine, who may face more difficulty obtaining private loans, said Dr. Virginia Caine, president of the National Medical Association, which represents Black physicians and which advocates for health equity. In turn, she said, it will limit those students' ability to return to serve in their communities.
By 2037, the United States is expected to face a shortage of 187,130 physicians, including 87,150 primary care physicians such as internists and pediatricians who play a pivotal role in the early detection and management of chronic disease, according to the federal Health Resources and Services Administration. (There are currently 933,788 professionally active physicians.)
About 75 million Americans live in areas where it is difficult to get access to primary care. The ratio of primary care providers is projected to decline to 76.8 per 100,000 people by 2037, from 81.6 per 100,000 in 2022.

MacDonough serves as a referee to decide what fits with Senate budget rules. The various provisions which MacDonough fillets from the bill are called "Byrd droppings." Anyway, despite the cornpone Senate humor, the process offstage on Sunday is crucial to the process. What's ruled in or out could bolster chances of passing the bill – or kill it. We will start to get information about what is ruled in or out over the weekend and stretching into Monday. Those policy details will be critical. So watch for leaks and other information to dribble out beginning on Sunday. This process will roll into early next week with an initial vote to begin the process mid-week. It will culminate with a round-the-clock voting session (known as a "vote-a-rama") late next week. Then the measure must go back to the House. That's because the Senate will inevitably change the bill. The House and Senate must be aligned before the bill can go to the president's desk. And this is why the deadline to finish the bill by July 4 may slip.

A new Fox News survey finds more than half, 52%, think it is important to investigate whether Biden advisors used an autopen without the president's awareness, while 46% say it's time to move on. House Oversight Committee Chair James Comer is pursuing investigations against former Biden administration staff for allegedly covering up Biden's mental decline and using an autopen for executive actions. Views are split on whether advisors should be scrutinized for making key presidential decisions on behalf of Biden (50% investigate vs. 49% move on), while a majority opposes investigating whether Biden advisors covered up his decline (44% investigate vs. 55% move on). Since his first campaign for president 10 years ago, Trump has excelled at appearing to favor both sides of the same issue, allowing supporters to hear what they want to hear, whether he's talking about tariffs, TikTok, abortion, tax cuts, or more. But the prospect that the United States might join Israel in bombing Iran is testing his ability to embrace dueling positions with little to no political cost. Some of Trump's most ardent supporters — those who defended him during multiple investigations and ultimately returned him to the White House — are ripping one another to shreds over the idea and at times lashing out at Trump as well. Advertisement The war in Iran is exactly the kind of Middle East entanglement that Trump's anti-interventionist base believed he was bitterly opposed to, because he repeatedly said he was. But he is also the same president who, in his first term, authorized missile strikes in Syria, after its leadership used chemical weapons on citizens, and the assassination of a top Iranian general, Qassem Soleimani — two actions he took pride in. 'I think I'm the one that decides that,' he told The Atlantic recently in response to criticism from one of his most vocal anti-interventionist supporters, Tucker Carlson, who said the president's support for Israel's fight in Iran ran against his 'America First' message. Trump was propelled to victory in the Republican primary in 2016 as an outsider, in part because he forcefully condemned the invasion of Iraq, authorized by the last Republican president more than a decade before, and the seemingly endless war that followed. Yet he said the United States should have taken the country's oil, and ran radio ads saying he would 'bomb the hell' out of the Islamic State group. He has said he supports businesses and also wants to deport the immigrant workforce that fuels parts of the economy. He wants to engage in mass deportation and also wants to sell visas for $5 million. He has celebrated the Supreme Court decision to overturn Roe v. Wade as a point of pride while also condemning Republican governors who signed bills banning most abortions after six weeks of pregnancy. He has both celebrated and criticized his own criminal justice reform bill of 2018. Despite the contradictions, Republicans for years have been united in support of Trump and what he says he wants, giving him a benefit of the doubt that few, if any, career politicians have ever received. Even when most elected Republicans held Trump at a distance after the deadly attack on the Capitol by a pro-Trump mob on Jan. 6, 2021, Trump still had a tight grip on Republican primary voters. Trump, a celebrity known to the electorate for decades, has obscured long-standing and unresolved foreign policy divisions within the party dating back to the aftermath of President George W. Bush's push to invade Iraq. Advertisement But as Trump decides whether to plunge the United States into the heart of the Israel-Iran conflict, his core supporters are splintering. Trump's announcement Thursday that he could take up to two weeks to decide did not sit well with some of his most hawkish supporters. On social media, Fox News host Mark Levin began a lengthy post by suggesting that the president was being pulled back from what he actually wants to do. 'LET TRUMP BE TRUMP!' Levin wrote. 'We got our answer. Iran says no unconditional surrender. Again. And again. And again. They cheat and lie and kill. They're TERRORISTS!' His anti-interventionist supporters, meanwhile, have been equally alarmed by what he might decide to do. 'Anyone slobbering for the U.S. to become fully involved in the Israel/Iran war is not America First/MAGA,' Representative Marjorie Taylor Greene, Republican of Georgia, posted on social media over the weekend. Trump's advisers say that on the Israel-Iran conflict in particular, the president is dealing with a fast-moving, complicated situation that does not lend itself to simple, black-and-white solutions, despite the fact that he has consistently campaigned that way. 'President Trump considers the nuances of every issue but ultimately takes decisive action to directly benefit American families,' said Anna Kelly, a White House spokesperson. 'The American people trust this president to make the right decisions,' she said, adding that he 'started the Make America Great Again movement because he represents a new leadership that puts Americans first.' But in 2025, Trump is not the only one who can command media attention. Carlson is no longer on Fox News, but he has a show that streams on the social platform X and is a leading voice among foreign policy 'restrainers' who have argued that Trump would be acting against his own movement should he strike Iran. Advertisement Steve Bannon, an adviser who was exiled from the White House in the first year of Trump's first term, has become one of the dominant voices among the MAGA faithful with his 'War Room' podcast, delivering the same message as Carlson. Yet Trump has found that many of his allies will ultimately come back to him, despite unhappiness with some of his decisions.
