Early Surgical Referral Key in Pharmacoresistant Epilepsy

Early Surgical Referral Key in Pharmacoresistant Epilepsy

Medscape3 days ago

OTTAWA — Surgery can achieve improvements in seizure activity in patients (particularly younger patients) with epilepsy that is refractory to drug therapies, according to a presentation at the Canadian Neurological Sciences Federation (CNSF) Congress 2025.
Roy Dudley, MD, PhD, a pediatric neurosurgeon at Montreal Children's Hospital and Montreal Neurological Institute and associate professor of pediatric surgery at McGill University, Montreal, underlined the importance of early referral for surgery.
An ad hoc task force of the International League Against Epilepsy defined drug-resistant epilepsy as the persistence of seizures despite at least two syndrome-adapted antiseizure drugs used at efficacious daily doses. Many patients with pharmacoresistant epilepsy have tried more than two medications, said Dudley.
Age influences responsiveness to surgery, and very young pediatric patients are good surgical candidates because of their neuroplasticity, he stressed.
'Pretty aggressive surgery, such as an anatomical hemispherectomy, can be performed in a child,' said Dudley, noting a case of a 2-year-old child who became seizure-free after such a surgery. 'The goal is to make a child seizure-free, but if we can decrease seizures significantly, that is important. If a young child continues to have seizures, their neurological development will be poor.'
Evolution in Technology
Citing published research, Dudley pointed out that intraoperative MRI (iMRI) has not been helpful in surgical cases where lesions are poorly defined and the boundaries of the lesions are not apparent.
'These are lesions that extend beyond what is seen on iMRI,' he said. 'The iMRI does not help you to see these lesions, which is what drove us to using new tools.'
One of those new tools is robot-assisted stereoelectroencephalography (SEEG), which accurately pinpoints the locations from which epileptic seizures originate. 'Using a robot makes it more precise to implant the electrodes because human error is taken out of the procedure,' said Dudley. Robot-assisted SEEG pinpoints seizure locations more rapidly.
Other tools that are currently used for epilepsy surgery include magnetoencephalography, EEG, functional MRI, and advanced MRI voxel-based post-processing.
Pool of Surgical Candidates
Gerald A. Grant, MD, Allan H. Friedman Distinguished Professor of Neurosurgery and chair of Neurosurgery at Duke University School of Medicine in Durham, North Carolina, agreed that younger age increases the benefits of surgery in a patient with drug-resistant epilepsy.
'There is a greater chance for plasticity or rewiring of the brain if we treat as young as possible, which is also why it is so gratifying as a pediatric neurosurgeon to treat children with severe epilepsy, since we can have such enormous impact on their lives,' he told Medscape Medical News. 'We have advocated to identify the children who might benefit most from surgical intervention as young as possible, since the longer we wait, the greater the impact on long-term neurological development.'
Grant noted that working with neurologists is key to expanding the pool of candidates for epilepsy surgery. 'If a child fails two or more seizure medications, they may be a candidate for surgical intervention and should be referred early to a comprehensive epilepsy center. Partnering with the community neurologists is key, since there are many children with severe medically refractory epilepsy who are surgical candidates who, unfortunately, are not referred until much later in their course or even into adulthood.'
Artificial intelligence (AI) also might play a role in referrals to surgery. 'We are now testing an AI tool embedded in our electronic medical record at Duke to look at every child or adult getting evaluated by a neurologist and flagging the chart if, based on the AI algorithm, they potentially could benefit from a referral for surgical intervention,' said Grant.

Measles 'out of control,' experts warn, as Alberta case counts surpass 1,000

Alberta's measles outbreaks have now eclipsed the 1,000-case mark and infectious disease specialists are warning the virus is "impossible to contain," given the current level of transmission. The province reported another 24 cases on Friday, including 14 in the north zone, nine in the south and one in the Edmonton zone. This brings the total confirmed cases since the outbreaks began in March to 1,020. "It is a very grim milestone," said Dr. Karina Top, a pediatric infectious disease physician at the Stollery Children's Hospital in Edmonton, which has been treating children with measles. "I'm very worried we're going to see more hospitalizations and some deaths soon because we know the death rate is about one to two per thousand. So it's likely that we're going to see that and that will be a very tragic day." Measles is highly contagious and can lead to serious complications including pneumonia, brain inflammation — which can trigger seizures, deafness and brain damage — as well as premature delivery. A premature baby who was born with measles died in Ontario recently. And another young child died of measles in that province last year. Doctors warn there are severe long-term consequences as well, including immune system impacts and a degenerative neurological condition that occurs seven to ten years after an initial measles infection. It is rare but nearly always fatal. 'Out of control' Alberta's case count has more than doubled in the last month. On May 20, a total of 486 cases had been confirmed. "This is out of control," said Top. The hardest hit areas are the south, central and north zones, where there are some very low vaccination rates among young children. Case counts have been rising quickly in the north zone, which has now topped 200 cases. The south zone has confirmed 681 cases and the central zone has 105. And health officials are warning the virus is more widespread in those areas than case counts reflect. "Due to the number of people in these areas who may not be immune to measles, it's likely that some cases are going undetected or unreported," the Alberta government's measles web page states. "I think we really need to be doing everything we can to engage with those communities that are affected and community leaders to see how best we can work with them to try to get people vaccinated to help contain this infection," said Top. "And [we need to] get everyone else up to date so that if there are other cases introduced in Edmonton, Calgary or elsewhere in the province, it can be contained more quickly." University of Calgary infectious disease physician Dr. Dan Gregson is also worried about Alberta's surging measles cases. "This transmission is just out of the box. It's impossible to contain at the present time," he said. "The risk, right now, of your child getting measles in Alberta if they're not immunized is pretty high." Gregson is urging parents who have opted not to immunize their children to rethink that decision. "If you had made a decision not to vaccinate because measles was not circulating, that's no longer true. We don't have herd immunity. There's measles circulating in the community. And the best way to prevent your child from having a complication would be to get them vaccinated," he said. The vast majority of Alberta's measles cases are among the unimmunized. The most recent data shows 85 Albertans have been hospitalized due to this year's outbreaks, including 14 who have ended up in intensive care. "Most of these hospitalizations are due to patients having pneumonia — they're short of breath, they have to wear oxygen. Some of them are due to brain inflammation. [It's] not a pleasant thing for the parents or the child to have to go through," said Gregson. In Edmonton, where the Stollery Children's Hospital is treating the sickest patients from northern Alberta, Top said the outbreaks are taking a toll. "It is distressing as a pediatrician to see children that are sick from a disease that is completely vaccine preventable." According to provincial data, as of Friday there was one Albertan in ICU due to measles. No deaths have been reported.

Vitalhub concludes Induction Healthcare acquisition

Canadian software company Vitalhub has announced the completion of its previously announced acquisition of Induction Healthcare Group. The acquisition was finalised through a court-sanctioned scheme of arrangement under Part 26 of the Companies Act 2006. Vitalhub consented to the acquisition terms in April, agreeing to a cash purchase that values Induction Healthcare at approximately £9.7m ($13.06m). Induction is known for its primary products, Zesty and Attend Anywhere, both of which are designed to enhance the patient journey and streamline hospital operations. Zesty serves as a patient engagement platform, while Attend Anywhere facilitates secure video consultations for healthcare organisations. For the six months ending on 30 September 2024, Induction reported revenues of £5.4m, with a gross margin of 78.2% and an adjusted EBITDA loss of £0.9m. Zesty generated £2m and Attend Anywhere reported £3.2m in revenues, with a small portion from discontinued operations. During acquisition, Zesty's annual recurring revenue stood at £2.2m, and Attend Anywhere's virtual care recurring revenue was at £4.6m. VitalHub CEO Dan Matlow said: 'We see the Induction product set, in particular the Zesty platform, as highly complementary to our current suite of solutions. 'We believe our expanded end-to-end offering will enhance the value to our healthcare partners, improving the end-user experience and patient outcomes.' Vitalhub's acquisition of Induction for £0.10 per share aligns with its growth strategy, which includes both organic development and an aggressive approach to mergers and acquisitions. The company's product suite serves more than 1,000 clients across the UK, Canada, and other regions, with a workforce of over 500 employees worldwide. Induction's software solutions are designed to offer healthcare providers and administrators the ability to deliver care both remotely and face-to-face. This acquisition not only broadens Vitalhub's offerings but also aims to give communities more flexibility and ease of access to healthcare services.
