
#48: The science-backed diet-and-fasting blueprint for a longer, healthier life

'What's the most effective diet and fasting framework that helps improve both lifespan and health span?'
This is a question I get asked a lot, and one that I have spent many years trying to research and experiment with. With so many diets and fasting strategies out there, it's hard to separate trends from what's actually backed by science. In today's post #48, I provide my conclusions and recommendations, along with the clinical evidence supporting them. As always, while each of our bodies react differently and therefore need to be optimized for accordingly, the Evidence Based Medicine (EBM) studies I quote have been done across large populations. For those that want the conclusion without reading further, here they are: a) base your meals on Mediterranean principles, b) eat in a 10 AM–7 PM window, c) add in a once or twice a month 24 hour fast, and d) avoid extremes.
A Quick Note on Evidence Based Medicine (EBM)
Evidence-Based Medicine or EBM—the practice of making health decisions based on the best available scientific research out there. But not all the millions of studies published are equal, so scientists rank them by strength.
Sort A = High-quality evidence (large, consistent clinical trials or meta-analyses). Most trustworthy.
Sort B = Moderate-quality evidence (some good studies, but with mixed results or smaller scale).
Sort C = Low-quality evidence (based on expert opinion, early trials, or animal studies).
There is also a metric called I² (I-squared), which tells us how much the results vary across the numerous studies in a meta-analysis.
I² < 25% means the different studies mostly agree = reliable evidence.
I² > 50% means the different studies had different results = use with caution.
Together, Sort level and I² help us separate what's truly proven from what's still speculative. This combination—EBM Sort A with I² < 25%—is what I consider the gold standard. When evidence meets that bar, it makes my short list.
The clear winner: Mediterranean diet
EBM Sort A, I² = 9%
If there's one eating pattern that consistently wins across cultures and decades of studies, it's the Mediterranean diet.
In a meta-analysis of over 1.5 million people, those who followed this diet saw a 9% reduction in all-cause mortality. The PREDIMED trial, one of the most robust dietary studies ever conducted, showed a 30% drop in cardiovascular events in participants eating a Mediterranean-style diet enriched with olive oil or nuts.
The key ingredients of a Mediterranean diet include vegetables, legumes, olive oil, fish, nuts, whole grains—and very little sugar or processed food. It's sustainable, flavorful, and deeply rooted in how some of the world's healthiest populations eat. Coupled with intermittent fasting (10am to 7pm eating window)
EBM Sort A, I² = 22%
Now let's talk about when you eat. I personally follow intermittent fasting (IF) with a 10 AM to 7 PM eating window, giving my body ~15 hours of fasting each night. This schedule fits my lifestyle while tapping into powerful metabolic benefits.
In a 2020 meta-analysis with I² = 22%, IF was shown to significantly reduce fasting glucose, insulin, body weight, and inflammation—even without reducing calorie intake. That's a big deal. IF works not just because you eat less, but because you give your body time to rest, repair, and rebalance.
IF activates AMPK (your cell's energy regulator) and suppresses mTOR (your body's growth and aging switch), while enhancing autophagy, the process of cleaning out damaged cellular parts. These pathways are at the core of what we know about aging and healthspan. If these words sound too scientific, don't worry about it. I'll be doing a post on explaining these in simple terms because an understanding of these mechanisms help with understanding how our bodies behave during fasting vs. when we are well fed, and both have important roles to play in longevity and healthspan.
And/or calorie restriction (without malnutrition) EBM sort A, I² < 20% (CALERIE Trial)
Calorie restriction (CR)—eating about 10%–15% fewer calories than your body burns—has been shown to extend lifespan in every species studied. In humans, the CALERIE trial found that two years of mild CR improved insulin sensitivity, lowered inflammation, and improved cardiovascular biomarkers—even among healthy, non-obese individuals.
But it comes with caveats: reduced energy, muscle loss, and risk of nutrient deficiencies if you're not careful. For those of us over 50, it's essential to prioritize protein and strength training if exploring this strategy. Alternatively, intermittent fasting can deliver many of the same benefits without constant calorie counting.
Diets with weak or mixed evidence Some popular diets just don't hold up under rigorous scientific review. I cover them as well so as to provide the rationale behind why they don't make the cut. Here's how they rank:
1. Ketogenic diet EBM sort B, I² = 38%
Keto has proven benefits for epilepsy and can improve insulin resistance and weight loss in type 2 diabetes. But for healthy individuals focused on longevity, the evidence is thin. No long-term studies show a reduction in mortality or aging-related decline. High saturated fat intake, low fiber, and nutrient gaps are concerns. In short: therapeutic, but not necessarily longevity-enhancing for most reasonably healthy individuals.
2. Paleo/ carnivore diets EBM Sort B, I² > 40%
These diets cut out processed food, which is good—but they also eliminate fiber-rich, longevity-promoting foods like legumes and whole grains. Most studies are short-term and observational. No strong Randomized Controlled Trials or RCTs support these diets for long-term healthspan.
3. Ultra-processed vegan diets EBM sort B, I² = 35%–50%
Whole-food plant-based diets are associated with lower mortality and disease risk—when done right. But highly processed vegan diets (mock meats, sugary snacks) don't offer the same benefits. Quality matters more than the label. If you are on a vegan diet, make sure you keep a close eye on the quality of your carbs.
My summarized recommendations:
Here's what I follow—and recommend—if health span and longevity are your goals:
Base your meals on Mediterranean principles: veggies, legumes, olive oil, fish, whole grains, and nuts. I don't eat much fish but I do have chicken (grilled usually) and some amount of unflavored, unsweetened yogurt. Eat in a 10am–7pm window to give your body time to repair overnight. While I follow a 15 hour fasting window, even a 12-14 hour window is fine. Make sure it's practical to fit within your lifestyle, but avoid eating too much after sunset, since our circadian rhythm is a big driver of hormone balance. Add in a once-a-month or twice-a-month 24 hour fast to trigger deeper autophagy and mitochondrial rejuvenation. It is critical to not overdo it to ensure no muscle mass loss, and to avoid thyroid dysregulation. Avoid extremes. Diets that exclude major food groups often don't stand up to long-term evidence.
Final thought
Longevity is about combining the best of what modern science and ancestral wisdom agree on—and doing it consistently. Eat well. Eat smart. Give your body time to recover. And build strength—this will stand you in good stead for decades to come.

In a significant milestone, Hindustan Aeronautics Limited (HAL) was on Friday declared the winner of the bid for the transfer of technology of the Small Satellite Launch Vehicle (SSLV), the Indian Space Research Organisation's (ISRO) rocket to place satellites up to 500 kg in low-earth orbit, edging out two consortia. According to a PTI report, fighter jet manufacturer HAL was the standalone bidder for the coveted contract to build the ISRO-designed rocket and was pitted against the two consortia – one led by Alpha Design Technologies, backed by the Adani Group, and the other led by Hyderabad-based Bharat Dynamics Limited. In a message on X, HAL said that it will be responsible for absorbing, manufacturing and commercialising SSLV technology. 'HAL's selection for the SSLV ToT will enable indigenous production and further development of small satellite launch capabilities, opening up new partnerships with domestic and international satellite operators,' Dr DK Sunil, Chairman and Managing Director of HAL, said. HAL presented the winning bid of ₹ 511 crore to emerge as the sole manufacturer of the SSLV after the completion of the process of transfer of technology that is expected to take place over the next two years, the PTI report said. "Under this technology-transfer agreement, HAL will have the capability to independently build, own and commercialise SSLV launches," Pawan Kumar Goenka, chairman, Indian National Space Promotion and Authorisation Centre (INSPACe), told a press conference in New Delhi. HAL will be the third company to build rockets after space sector start-ups Skyroot Aerospace and Agnikul Cosmos, the report added. Goenka said nine companies had evinced interest in the transfer of technology of the SSLV, of which three were rejected. Of the remaining six, three chose not to apply. "The SSLV technology transfer marks a pivotal moment in India's transformative commercial space segment, as this is one of the first instances of a space agency transferring complete launch vehicle technology to a company," he said. Goenka said that ISRO will handhold HAL and assist it in building two prototype rockets over the next two years. After that, the state-owned company is expected to independently produce six to 10 SSLVs every year, depending on the demand. "HAL will be free to improve on the design and select its own vendors from the third rocket after the two-year period," Goenka said. According to HAL, SSLV is a compact, cost-effective, and highly flexible launch vehicle developed by ISRO to address the rapidly growing demand for launching small satellites into Low Earth Orbit. The SSLV was developed by the ISRO to launch small satellites into low-earth orbit at a shorter notice, a capability required by the defence forces in times of emergencies. "This collaboration marks a significant step towards strengthening India's commercial satellite-launch capabilities and more specifically, in enabling Indian industry in realising this SSLV," Radhakrishnan Durairaj, Chairman and Managing Director of New Space India Limited (NSIL), said. According to the PTI report, the move is part of efforts to allow the ISRO to focus more on research and hive off the routine activities of launching satellites, using proven launch vehicles to the industry. "We are looking forward to working closely under the ISRO and IN-SPACe's guidance to progress in phases and realise the end objectives. We are confident of steering a cohesive ecosystem that enables more small satellite launches from India's ports," DK Sunil, CMD, HAL, said. The technology-transfer agreement will be signed among HAL, NSIL, ISRO and IN-SPACe.

The email shows the department will no longer cover the costs for those transgender veterans preparing for gender dysphoria treatment and seeking cryopreservation. Dsyphoria treatment is a process in which eggs or sperm (gametes) are frozen and stored to be thawed for later since Donald Trump has returned to office, he has taken a slew of actions regarding transgender people, including signing an executive order stating that there are only two unchangeable sexes and another attempting to ban transgender women from competing in women's sports. He also signed an order aimed at restricting access to puberty blockers and other gender care for people under age 19 with some of his actions sparking legal challenges in the March, it was announced in the VA specifically that the department began phasing out treatment for gender dysphoria following Trump's executive order recognizing only two sexes. However, the VA stated that transgender veterans would still have access to VA health care the email now indicates a shift from the previous policy. "On yesterday's (6/16/2025) National Fertility Interdisciplinary Teams Office Hours call, it was announced that VA will no longer pay for cryopreservation of gametes for a medical indication if it involves transgender veterans seeking cryopreservation in preparation for treatment of Gender Dysphoria or Gender Identity Disorder. This will reportedly affect new authorizations and will not disrupt cryopreservation that has already been initiated," the email viewed by NewsWeek added that according to another senior official in the Veterans Health Administration "the change is reportedly already in effect and an official announcement will be forthcoming" and advised the department to "hold on authorizing any new cryopreservation for medical indications related to gender dysphoria/gender identity disorder" while waiting for further guidance."We want to avoid a situation where veterans could be told they are approved for cryo, only to find out after the fact that VA cannot pay for care they received," the email Keith, the founder and executive director of the Michigan-based transgender charity Stand With Trans, said the policy was "discrimination.""Clearly, the rights of the trans community are the target of attack," she told Newsweek. "It doesn't matter whether someone is an adult, a minor, a veteran, a professional, or other, if one is trans identified, they are seeing rights being taken away one by one. It is deplorable. If one class of people can receive treatment paid for with their benefits, then other classes of people should receive the same. This is why we have the equal protection clause in our Constitution."No one should be excluded from a particular treatment because they are transgender. This is the very definition of discrimination."This comes days after the Trump administration denied reporting by The Guardian that said new VA hospitals could refuse care to veterans based on factors like marital status and political affiliation due to an executive order by President Guardian earlier Monday published a report saying VA hospitals are implementing new rules in response to Trump's executive order in January, which would permit workers to deny care to veterans based on characteristics not protected by federal House deputy press secretary Anna Kelly adamantly denied The Guardian's reporting. Writing on social platform X, Kelly addressed the article's writer, Aaron Glantz, saying, 'Aaron, this is a totally FALSE story that The Guardian should retract immediately. Fearmongering with our Veterans to try to score clicks for your failing 'news outlet' is pathetic and shameful.'

Pics: Nearby Sculptor Galaxy Revealed In Ultra-Detailed Galactic Image
Washington: The Sculptor galaxy is similar in many respects to our Milky Way. It is about the same size and mass, with a similar spiral structure. But while it is impossible to get a full view of the Milky Way from the vantage point of Earth because we are inside the galaxy, Sculptor is perfectly positioned for a good look. Astronomers have done just that, releasing an ultra-detailed image of the Sculptor galaxy on Wednesday obtained with 50 hours of observations using one of the world's biggest telescopes, the European Southern Observatory's Chile-based Very Large Telescope. The image shows Sculptor, also called NGC 253, in around 4,000 different colors, each corresponding to a specific wavelength in the optical spectrum. Because various galactic components emit light differently across the spectrum, the observations are providing information at unprecedented detail on the inner workings of an entire galaxy, from star formation to the motion of interstellar gas on large scales. Conventional images in astronomy offer only a handful of colors, providing less information. The researchers used the telescope's Multi Unit Spectroscopic Explorer, or MUSE, instrument. "NGC 253 is close enough that we can observe it in remarkable detail with MUSE, yet far enough that we can still see the entire galaxy in a single field of view," said astronomer Enrico Congiu, a fellow at the European Southern Observatory in Santiago, and lead author of research being published in the journal Astronomy & Astrophysics. "In the Milky Way, we can achieve extremely high resolution, but we lack a global view since we're inside it. For more distant galaxies, we can get a global view, but not the fine detail. That's why NGC 253 is such a perfect target: it acts as a bridge between the ultra-detailed studies of the Milky Way and the large-scale studies of more distant galaxies. It gives us a rare opportunity to connect the small-scale physics with the big-picture view," Congiu said. Sculptor is about 11 million light-years from Earth, making it one of the closest big galaxies to the Milky Way. A light-year is the distance light travels in a year, 5.9 trillion miles (9.5 trillion km). Like the Milky Way, it is a barred spiral galaxy, meaning it has an elongated structure extending from its nucleus, with spiral arms extending from the ends of the bar. Its diameter of about 88,000 light-years is similar to the Milky Way's, as is its total mass. One major difference is Sculptor's rate of new star formation, estimated to be two to three times greater than that of the Milky Way. Nearly 30% of this star formation is happening near the galaxy's nucleus in what is called a starburst region, as revealed in colorful emissions shown in the new image. The observations have given information on a wide range of properties such as the motion, age and chemical composition of stars and the movement of interstellar gas, an important component of any galaxy. "Since the light from stars is typically bluer if the stars are young or redder if the stars are old, having thousands of colors lets us learn a lot about what stars and populations of stars exist in the galaxy," said astronomer Kathryn Kreckel of Heidelberg University in Germany, a study co-author. "Similarly for the gas, it glows in specific bright emission lines at very specific colors, and tells us about the different elements that exist in the gas, and what is causing it to glow," Kreckel said. The initial research being published from the observations involves planetary nebulae, which are luminous clouds of gas and dust expelled by certain dying stars. Despite their name, they have nothing to do with planets. These nebulae can help astronomers measure the precise distances of faraway galaxies. The researchers marveled at the scientific and aesthetic value of the new view of Sculptor. "I personally find these images amazing," Congiu said. "What amazes me the most is that every time I look at them, I notice something new - another nebula, a splash of unexpected color or some subtle structure that hints at the incredible physics behind it all."
