Heads up: How screen time and slouching could lead you to Dropped Head Syndrome

Little do most of us realise that our everyday habits such as constant slouching, skipping regular physical activity, and endless hours spent looking down at our phones or poorly positioned computer screens, might be coming at a hidden cost. One that could literally make your head drop forward and stay that way.
This is the uncomfortable reality of Dropped Head Syndrome (DHS), a condition caused by weakness in the neck's extensor muscles, which can lead to a forward slump of the head until the chin rests on the chest. Left untreated, this posture isn't just a passing ache, it can become permanent.
And with Malaysians ranking among the world's top in daily screen time, clocking in an average of eight hours a day, four of which are spent on mobile devices, we might be unknowingly setting ourselves up for it.
THE SPINE DOESN'T LIE
According to Consultant Spine Surgeon Dr Lim Kang Kai from Sunway Medical Centre, a combination of bad habits is contributing to poor spinal health.
'Even simple daily activities such as getting in and out of bed, driving, bending down to wear your shoes, or picking an object from the floor can be damaging if the proper techniques are not observed," he explained.
What's worse, he added, is how lack of exercise and weak core muscles tend to 'drive the nail in the coffin.'
DHS, he explained, is caused by severe weakness or deficiency in the neck's back muscles, the extensor muscles. This causes the head to involuntarily fall forward, making it difficult to maintain an upright position. Consultant Spine Surgeon Dr Lim Kang Kai
The signs? A head that's tilted downward, a painless chin-on-chest posture, and a neck that appears elongated. People with DHS often struggle to lift their heads while sitting or standing, although lying down can temporarily relieve the position.
Dr Lim said that DHS is classified as a neuromuscular disease, similar to conditions like Parkinson's, myasthenia gravis, ALS (amyotrophic lateral sclerosis), polymyositis, and genetic myopathies. It can also be linked to degenerative causes such as cervical spondylosis or age-related muscle loss (sarcopenia).
'When the cause is unknown, it is called isolated neck extensor myopathy,' he added.
While DHS is more common among the elderly, Dr Lim warned that it isn't age-exclusive. The symptoms may appear suddenly or progress over months and years, depending on the root cause.
'Younger individuals may be affected more rapidly due to immune-mediated neuromuscular conditions or acute neurological causes, compared to older individuals whose cases are often linked to chronic conditions. although the reverse can be true for both age groups.'
TREAT FIRST
Fortunately, DHS can often be managed through non-surgical treatments at the early stage.
'A supportive neck collar coupled with physical therapy may be of tremendous help. In certain inflammatory conditions, medications may be used to reduce muscle inflammation. Immunotherapy or other disease-specific medications may be part of the treatment for DHS,' said Dr Lim.
But in more stubborn or severe cases, surgical intervention like spinal fusion surgery may be necessary to correct deformity and restore proper head posture.
Dr Lim also clarified that DHS should not be confused with everyday neck muscle sprains or trauma, although they might look similar, the underlying causes and treatments differ.
'While both conditions can involve neck weakness, their causes, mechanisms, and treatments are very different,' he said, pointing out that DHS is usually a gradual process linked to neuromuscular issues, while sports injuries happen suddenly and are treated with rest, NSAIDs, and physiotherapy.
When it comes to recovery, age plays a role. 'Younger people have the capacity to recover faster, while the elderly heal slower,' Dr Lim noted. Rehabilitation Medicine Specialist Dr Foong Chee Choong
REHABILITATION
For those already battling the condition, Rehabilitation Medicine Specialist Dr Foong Chee Choong, also from Sunway Medical Centre, explained that therapy focuses on restoring flexibility and strength, particularly in the neck extensor muscles, torso, and hips to improve posture.
Patients with severe neck weakness might benefit from wearing a cervical collar to maintain proper head alignment.
'Individuals with DHS who experience neck pain may also benefit from physiotherapy modalities such as heat therapy and electrotherapy for pain relief,' said Dr Foong. However, he cautioned that the effectiveness of these treatments largely depends on what's causing the condition in the first place.
Those with fixed flexion deformity and failed rehab outcomes may need to consult a spine surgeon for possible corrective surgery.
Echoing Dr Lim, Dr Foong said DHS is mostly triggered by neurological diseases and degenerative spinal issues, both of which are difficult to prevent. But lifestyle matters.
'Excessive use of smartphones and other electronics could aggravate the issue.
'Fixed flexion deformity of the neck is preventable by practising proper body mechanics and regular neck exercises," he said.
Still, he noted, poor posture and improper sleeping positions more commonly cause neck and back pain, rather than full-blown DHS.
Ultimately, whether DHS is reversible or not hinges on its cause, early intervention and proper diagnosis is crucial.
The message is clear: our posture, daily movements, and tech habits can shape or misshape our bodies in lasting ways. While most of us worry about phone addiction, we may be ignoring its physical toll.
So maybe it's time to straighten up, stretch out, and look up — literally.

'Acne is a multifactorial skin condition that extends far beyond visible lesions. A holistic approach not only improves outcomes but also addresses the emotional burden carried by many patients,' said Consultant Dermatologist Dr Teeba Raja, who also serves as president of the Malaysian Allergic Contact Dermatitis Subcommittee. She explained that emerging research has drawn attention to the gut-skin axis, where imbalances in gut bacteria – known as gut dysbiosis – can drive skin inflammation. 'Increased intestinal permeability, or 'leaky gut', allows endotoxins such as lipopolysaccharides to enter the bloodstream and trigger inflammatory responses in the skin. 'While gut-directed therapies are not yet part of standard acne treatment, patients with both acne and gastrointestinal symptoms may benefit from addressing underlying gut issues,' she said. Teeba also highlighted the role of diet, noting that high-glycaemic foods and dairy have been linked to acne severity. 'Low-glycaemic diets help by lowering insulin and IGF-1 levels – hormones that stimulate oil production and skin cell growth. 'Dairy, especially skim milk and whey protein, can aggravate acne in some individuals due to its hormonal effects.' She added that probiotics, particularly strains such as Lactobacillus and Bifidobacterium, may help reduce skin inflammation, but should be seen as complementary rather than primary treatments. Mental health is another critical yet often overlooked aspect of acne care. 'Many patients experience depression, anxiety and social withdrawal due to acne. Some studies have found it to be more psychologically distressing than chronic conditions like asthma or diabetes. 'In Malaysia, adolescent surveys show acne is a common cause of emotional distress and social stigma.' Teeba called for routine mental health screening during dermatology consultations. 'We should normalise checking in on patients' emotional well-being. Tools such as dermatology-specific quality-of-life questionnaires can help identify those who need psychological support.' She recalled one case involving a 19-year-old male with severe nodulocystic acne. 'After years of failed treatments, he was started on isotretinoin. In six months, not only did his skin improve dramatically, but his self-esteem and overall quality of life returned. It was a reminder that acne care can be life-changing when done right.' Teeba also warned against common self-treatment mistakes, such as over-cleansing, pimple-popping and frequent product switching. 'These habits worsen inflammation, damage the skin barrier and increase the risk of scarring. Patients should be educated to follow a consistent, gentle routine and apply treatments across the entire acne-prone area, not just on visible spots.' She pointed out that social media has become a double-edged sword in acne management. 'Teens are constantly exposed to filtered images and miracle-cure content, which creates unrealistic expectations. Some abandon effective treatments in favour of viral trends. We must teach patients to critically evaluate online content and follow credible sources.' While public awareness of acne in Malaysia is growing, stigma remains a persistent barrier. 'Studies show that over 76% of people view acne negatively in photographs, associating it with poor hygiene or unattractiveness. We need greater public education to dismantle these harmful myths.' Ultimately, she said acne treatment must extend beyond prescriptions.

At the peak of her career, Datuk Dr Florence Sinniah did something few dare to do – she walked away. Not for a higher post or early retirement, but to listen to the cries of people crushed by grief or despair. 'I left when I was doing my best work. My performance was consistently in the top tier. 'But I knew I had another calling,' said Florence, founder and chairman of Sneham Malaysia – a renowned toll-free suicide prevention helpline. From corporate corridors to hospital wards, Florence wore many hats. She has worked as a psychiatric nurse, a midwife and a senior human resources (HR) director in multinational companies. It was during her HR career – navigating the mental breakdowns of top-performing employees – that she discovered her purpose. 'One case that changed me was an engineer who broke down from the pressures of managing a global team. 'I realised then that I had the ability to connect with people deeply, and that the mental health of people cannot be an afterthought,' she said in an interview. That realisation planted the seed that would grow into Sneham, which in Tamil means 'love' or 'friendship'. Florence began volunteering with the Befrienders hotline in 2009, quietly taking the graveyard shift from midnight to 6am every Friday – just to be there for those in crisis. By 2018, she knew she had to go further. She resigned and launched Sneham in October that year. Today, Sneham is powered by 35 dedicated volunteers, supported by 48 members in total. It's more than a hotline – it's a lifeline for those trapped in darkness. 'In a world where emotional pain is often invisible, just having someone to listen without judgment can save a life,' she said. Since 2019, Sneham has answered 9,235 calls, responded to 2,292 WhatsApp messages, provided 128 in-person counselling sessions, and reached 193 schools and communities with outreach programmes. During the pandemic period, calls surged from 375 in 2019 to 1,885 in 2023, showing how isolation and despair gripped a higher number of people. But running a lifeline isn't easy. Florence is candid about the struggles. 'We're constantly working to expand our volunteer pool and secure enough funding to keep going,' she said. In 2020, Sneham was recognised globally with the Stars of Covid Award – one of 100 selected from over 1,600 nominations. Behind the numbers lies the beating heart of a woman who gave up comfort to sit with people in their darkest moments – simply to remind them that they matter. Meanwhile, the Star Golden Hearts Award (SGHA) returns for the 11th edition to honour everyday Malaysians whose compassion and selflessness have made a lasting impact on others. A light in the dark: Need for palliative care surges amid challenges, resource gaps

MIRI (June 22): Through its quiet but consistent efforts, the Palliative Care Association of Miri (PCAM) has, for the last two decades, helped many patients live with dignity, comfort and support in the face of life-limiting illnesses. The community-based organisation provides home care services, often reaching deep into remote and underserved areas like Batu Niah, Suai, Bakong, parts of Baram as well as Limbang and Lawas. Still, sustaining these services has never been easy. PCAM operates with a small team and limited financial resources, often without a full-time trained palliative care doctor. While community donations and volunteers provide essential support, the growing demand for its services is beginning to outpace what the association can realistically provide. These persistent challenges raise difficult questions about the long-term sustainability of grassroots palliative care – particularly in underserved regions like northern Sarawak. And yet, many still misunderstand what palliative care is all about. According to the World Health Organisation (WHO), palliative care aims to prevent and relieve health related suffering associated with life-threatening illness. It involves managing pain and symptoms, while also addressing psychological, social and spiritual needs. Melissa Desmond, one of PCAM's longest-serving nurses, said despite various outreach efforts, the public understanding of palliative care remains limited. 'People often think we only come in at the very end, but palliative care starts much earlier. It's about managing symptoms, supporting families, and giving people a sense of dignity through their journey,' she said. PCAM's struggles, even as it celebrates its 20th anniversary this year, mirror broader national challenges on the availability and development of palliative care services. Malaysia's growing elderly population and the increasing burden of chronic diseases are creating an urgent need for more accessible and better-resourced palliative services – not just in Sarawak, but nationwide. Although palliative care was introduced in Malaysia in the 1990s, the field remains under-resourced. This is becoming increasingly concerning as the country undergoes a demographic shift marked by a growing elderly population and rising prevalence of non-communicable diseases (NCDs). Melissa says despite various outreach efforts, the public understanding of palliative care remains limited. At the launch of the National Palliative Care Policy and Strategic Plan 2019-2030 in 2019, the Ministry of Health (MoH)'s then-national head of palliative medicine services, Dr Richard Lim, revealed that the number of Malaysians requiring palliative care is expected to more than triple by 2030 – from 71,675 cases in 2004 to 239,713. He also pointed out that less than 10 per cent of those in need are currently receiving proper care, citing the lack of trained personnel and facilities as major obstacles. In 2022, it was revealed by the then-Health Minister Khairy Jamaluddin that there were only 19 palliative specialists in the country – far below the estimated 218 required to meet national needs. The situation in East Malaysia is especially critical, with limited access to such services further deepening the inequality between urban and rural populations. A 2020 study by Su Lan Yang, previously a researcher at the National Institute of Health, and team projected that Sarawak is expected to surpass all other states by 2030 in terms of palliative care needs, with an estimated 35,065 cases. This projection highlights the growing burden of chronic, life-limiting illnesses in the state, including cardiovascular diseases and cancer. This increase in disease burden coincides with Sarawak's ageing population. According to the Department of Statistics Malaysia (DoSM), the state has one of the highest percentages of elderly citizens in the country. As of 2024, 13.4 per cent of Sarawak's population is aged 60 and above – second only to Perak (14.9 per cent). Nationally, Malaysia is on track to become an ageing nation by 2030, with the elderly population expected to exceed 15 per cent of the total. The DoSM data shows that 3.9 million Malaysians are now aged 60 and above, accounting for 11.6 per cent of the population, and 2.6 million people are aged 65 and above. The increasing number of senior citizens, many of whom are living longer due to better access to healthcare, has inevitably driven up the demand for palliative care services across the country. The need for comprehensive care is further driven by the country's NCD burden. The National Health and Morbidity Survey (NHMS) 2023 reported that more than half a million Malaysian adults are living with four major NCDs, namely diabetes, hypertension, high cholesterol, and obesity. Another 2.3 million are living with three of these conditions, increasing the likelihood of requiring long-term, multidisciplinary care. A recent study by the Institute for Public Health, under the National Institutes of Health, has also drawn attention to the high rate of premature deaths caused by NCDs, such as heart disease, cancer, stroke, and chronic respiratory diseases. These conditions are responsible for 74 per cent of deaths worldwide and disproportionately affect low- and middle-income countries like Malaysia. According to the study, Malaysia's probability of premature death due to NCDs declined from 20 per cent in 2017 to 18.8 per cent in 2019. However, the trend reversed following the Covid-19 pandemic, with the rate rising again to 19.4 per cent in 2021. It is projected to reach 20 per cent by 2030, mainly due to increasing deaths from cancer and diabetes. In comparison, Malaysia's premature mortality rate from NCDs is higher than that of Thailand (13.7 per cent) and Singapore (9.5 per cent), but lower than Indonesia (24.8 per cent) and the Philippines (24.5 per cent). These developments underscore the urgent need to expand and strengthen palliative care services across Malaysia, particularly in Sarawak. With the state poised to face one of the highest palliative care demands in the country, stakeholders stress the importance of investing in human resources, infrastructure, and outreach to ensure that no patient is left behind in accessing end-of-life care.
