What is rat lungworm disease and why are cases rising in dogs in Australia?

Cases of rat lungworm disease have been increasing in dogs on Australia's east coast over the past five years, according to new research from the University of Sydney. A very rare but sometimes deadly condition caused by worm larvae travelling up the food chain, the disease has been found in a small but increasing number of dogs in areas affected by heavy rain, and occasionally also in humans. So why are cases growing and how worried should we be?
The rat lungworm, or Angiostrongylus cantonensis, is a parasite that originated in south-east Asia but has spread to other parts of the world, including North America, Europe and Australia. The parasite's natural host is feral rats, but it can accidentally make its way to other animals through a complicated transmission process involving snails and slugs as intermediate hosts.
When it finds its way into humans and dogs, rat lungworm can cause a disease called neural angiostrongyliasis. In humans, it's diagnosed as a kind of meningitis, or eosinophilic meningoencephalitis. Symptoms in humans include headaches, a stiff neck, tingling or pain in the skin, fever, nausea and vomiting. Symptoms in dogs include hindlimb and tail paralysis, urinary incontinence and abnormal pain responses. The time between eating the infected slug or snail and getting sick is usually between one and three weeks.
University of Sydney doctoral researcher Phoebe Rivory, lead author of the recent paper, explained that it's not the parasite itself that does the damage but the body's attempts to fight it. 'In dogs and humans, the parasite enters the brain but rather than progressing to the lungs like it does in rats, it is killed in the brain by our own immune response. It is that overt immune response that causes severe headaches and sensations,' Rivory said.
New South Wales Health says it is rare for humans to develop meningitis in response to rat lungworm, and most people who contract rat lungworm have no symptoms at all, while others may have only mild, short-lived symptoms.
Rats infected with adult worms excrete larvae in their faeces, which is then ingested by snails and slugs. Dogs and people are most likely to contract the parasite when they deliberately or accidentally eat a raw snail or slug carrying the larvae, or unwashed vegetables, such as lettuce, contaminated with infected slime. The parasite is not contagious from person to person.
In a well-known case, Australian man Sam Ballard contracted eosinophilic meningoencephalitis in 2010 after being dared by his friends to eat a slug. He fell into a coma for 420 days, and acquired a brain injury. Ballard died in 2018, aged 29. In another well-known case, an American woman was found to have rat lungworm disease after multiple attempts to seek medical help for strange symptoms after a holiday to Thailand, Japan and Hawaii.
The University of Sydney researchers have been examining the prevalence of rat lungworm disease in dogs on the basis that they are a sentinel population for humans – that is, increased rates of the disease in dogs can be an early warning sign of human disease risk.
Researchers analysed 180 clinical samples taken between 2020 and 2024 from dogs suspected to have had the disease, and confirmed 93 cases, all in or around Sydney and Brisbane, and a 'gradual upward trend'. They also discovered a spike of 32 cases in 2022, correlating with high levels of rainfall, which tends to supercharge snail and slug proliferation.
Confirmed cases of the disease in dogs are still very low, restricted to NSW and Queensland, and have fallen since the 2022 spike identified by the researchers, although not to 2019 levels. The researchers suggest that educating communities about how to avoid infection, and what symptoms dog owners should look out for in their pets, may be worthwhile.
Rat lungworm is not a notifiable disease in NSW due to its rarity. In both NSW and Queensland, preventative anti-parasitic treatments for people, especially children, known to have ingested slugs or snails can be accessed through the health system.

A much-loved father and family member has died in hospital one year after he was hit by a car while riding his daughter's e-scooter. Isaac Craig was hit by a driver who ran a stop sign while testing out a new tyre on his daughter's e-scooter near his home in Townsville, Queensland, in March 2024. His injuries meant the Aussie dad was bedridden for most of the time and struggled to perform everyday tasks due to the lingering pain. Mr Craig suffered a heart attack last month with his wife Lisa forced to make the heartbreaking decision to turn off his life support one week later. Callum Albert, Mr Craig's stepson, wrote on a GoFundMe that it had been heartbreaking to watch him slowly deteriorate both mentally and physically. 'The last year of his life, he was almost completely bed-bound, he did have a wheelchair but it wasn't used often as he was in such pain and discomfort,' he said. 'He couldn't leave the house as it being an old Queenslander style home with stairs, made it almost impossible to comfortably get him up and down.' Mr Albert said on his stepfather's worst days, he would lock himself inside his bedroom to hide his worsening mental state from his family. He said the heart attack, while not directly linked to the e-scooter accident, came moments after he told his wife he was experiencing a mild anxiety attack. Mr Craig was without a flow of oxygen to his brain for 24 minutes before he was revived and rushed to Townsville Hospital. He was placed in ICU where doctors found he had little to no brain activity. His life support was turned off a week later. In yet another blow to his grieving family, Mr Albert said his mother is now facing the possibility of losing their family home. Mortgage payments have been paused while the family comes up with a solution. The GoFundMe raising funds for house expenses, cremation expenses, daily expenses of Mr Craig's wife and daughter and future bills has raised over $3,000. 'If donations do come, all of it will be given to his widowed wife and she will use it to best herself and their daughter Rebecca's current financial situation,' Mr Albert said. 'She has no idea about any of this (now she does) but I do know this will make her happy cry and to be relieved that even though it's not all the pressure, at least some is taken off her shoulders.' In an update, Mr Albert said his mother had been aware of the campaign and that she had received his stepfather's ashes. 'The support has been overwhelmingly substantial! Thank you to everyone who has helped so far,' he wrote.

Weight loss jab could be used to treat migraines

Weight loss jabs could cut the number of migraines sufferers experience by half, research suggests. The drugs, similar to Ozempic and Wegovy, also known as GLP-1 receptor agonists, work by mimicking the natural hormone which regulates blood sugar, appetite and digestion. Researchers have discovered, GLP-1 drug liraglutide, commonly used to treat diabetes, has the potential to significantly reduce migraine frequency. Migraines affect approximately 6 million people in the UK, according to the NHS. Attacks can last for three days causing pain, nausea, vomiting, dizziness and sensitivity to light, sound and smells. Researchers at the Headache Centre of the University of Naples gave 26 adults with obesity and chronic migraines the drug liraglutide. The study presented at the European Academy of Neurology (EAN) Congress 2025, found those who had the drug reported an average of 11 fewer headache days per month. Participants also experienced meaningful improvements in quality of life, work, study, and social functioning within just two weeks of taking the drug. 'Most patients felt better within the first two weeks and reported quality of life improved significantly', said lead researcher Dr Simone Braca. 'The benefit lasted for the full three-month observation period, even though weight loss was modest and statistically non-significant.' Growing evidence has linked subtle increases in intracranial pressure to migraine attacks. It's caused by a rise in the pressure of cerebrospinal fluid – which surrounds the brain and spinal cord. This increase in pressure can happen because of a severe head injury, stroke, a brain tumour or high blood pressure. But GLP-1-receptor agonists such as liraglutide reduce cerebrospinal fluid secretion and have already proved effective in treating idiopathic intracranial hypertension (IIH). Patients in the study were screened to exclude papilledema (optic disc swelling resulting from increased intracranial pressure) and sixth nerve palsy, ruling out IIH. Dr Braca and colleagues believe GLP-1 drugs may reduce the release of CGRP (calcitonin gene-related peptide), a key molecule behind migraines. 'We think that, by modulating cerebrospinal fluid pressure and reducing intracranial venous sinuses compression, these drugs produce a decrease in the release of calcitonin gene-related peptide (CGRP), a key migraine-promoting peptide', Dr Braca said. 'That would pose intracranial pressure control as a brand-new, pharmacologically targetable pathway.' Given liraglutide's established use in type 2 diabetes and obesity, it may represent a promising case of drug repurposing in neurology, study authors said.
