The mom of four sparked fresh surgery speculation after years of denial while arriving back to Los Angeles wearing an ab-baring look.
Kim's waist was on full display Wednesday, with the star rocking a cropped black halter top with matching leggings.
Kim, 44, was in Los Cabos with daughter North to ring in the tween's birthday, bringing along her niece Penelope Disick for the festivities.
Her sighting comes seven years after the star bragged about her waist measuring 24 inches and her hips measuring 39 inches - and three years after she revealed she lost 21 pounds to fit in Marilyn Monroe's iconic dress for the Met Gala.
She chose skintight bottoms paired with stiletto heels, adding the tiny backless top that showcased her toned abs.
The star loosely wore a jacket slung low over her shoulders, adding sunglasses to round out her summer look.
Kim wore the look for the plane ride from Los Cabos to Los Angeles, which she did via her private jet.
in 2018, Kim bragged about getting her waist down to 24 inches, and her hips to 39 - all thanks to her trainer at the time, Melissa Alcantara, via Elle magazine.
In 2017, Kim told mom Kris Jenner about her diet plans and working with Melissa: 'I'm going to start a really crazy diet. I was looking on Instagram and this body builder popped up on my page and she gained 70 pounds in her pregnancy. Her kid's like 6 now. She is like ripped,' the star said on an episode of Keeping Up With The Kardashians, per the outlet.
'Melissa is a trainer I found. She's super inspiring to me and I really want to meet up with her and see if she could train me. She could help direct my food and really get me on an amazing path.'
'If I was 10 percent her it would be life-changing. Like my whole arms and back. It's mostly food. I gotta have this lifestyle. I gotta try,' she said at the time.

In 2022, Kim revealed she has lost 21 pounds after overhauling her lifestyle to fit into Marilyn Monroe's dress for the Met Gala.
During an appearance on the Today show, Kim was asked how she lost the weight.
She had previously revealed she lost 16 pounds in three weeks by cutting out carbs and sugar in order to fit into the iconic dress - something which was heavily criticized for the message it sent.
'Yeah you know I looked at it like a role. I really wanted to wear this dress. It was really important to me. It actually taught me a lot about my lifestyle and my health and since then, afterwards, I continued to eat really healthy. I am down 21 pounds now. I'm not trying to lose any more weight but I have more energy than ever. I cut out so much sugar. A lot of junk food I was eating. I didn't even realize it. A lot of fried foods. And I completely changed my lifestyle,' Kim revealed.

The couple have remained tight-lipped about the upcoming nuptials but as the date nears, more details about the star-studded extravaganza have emerged. 9 9 9 The 61-year-old Amazon founder proposed to Sanchez, a former TV journalist, with a multi-million dollar ring on his yacht in May 2023. Mayor of Venice, Luigi Brugnaro, confirmed in March that the popular Italian city would host the couple and their guests for the highly-anticipated ceremony. He said the city was "extremely proud" the couple had chosen it for their wedding and called it "an honor that Bezos is coming to Venice". The happy couple appear to be keeping the celebration intimate with reports of around 200 guests being invited. But that relatively small list is thought to include some big names including celebrities, politicians, and business moguls. This is certainly the case if Sanchez's star-studded Parisian bachelorette party is anything to go by. The final hoorah before the "wedding of the century" was attended by the likes of Kim Kardashian, Kris Jenner, Katy Perry, and Eva Longoria. The couple are said to be using mainly local vendors to cater for the huge event. Around 80 percent of the wedding is to be locally sourced including from Rosa Salva, the city's oldest pastry maker and Laguna B, a studio famed for its hand-blown Murano glass. Rosa Salva, which has been in business since 1876, has been asked to provide an array of surprises for goody bags , Antonio Rosa Salva revealed. It is not known how Laguna B will contribute to the celebrations, with the business remaining tight-lipped. One source told The Mail that a celebrity wedding planner not connected to the event has been told guests are being given "opulent" gifts just to attend.
