The Data Tsunami In Today's Interoperable And AI World

Artificial intelligence (AI) systems have seemingly taken healthcare—and perhaps the world—by storm, showing great promise for revolutionizing care, research, workflows and more.
As the industry evaluates what's AI hype versus true potential, one thing about AI has become clear: It generates a massive amount of data—which for healthcare creates both opportunities and challenges.
Now that interoperability pipes are live and data is flowing between healthcare organizations, providers are managing a tsunami of incoming data. With AI and large language models (LLMs), it has become easier to generate even more data, which means the problem has just worsened. And unlike a regular tsunami, healthcare's giant wave of incoming data is not a one-time event. The data will continue to accumulate.
What are healthcare organizations to do to keep from drowning in all the data? Despite early hopes that AI might solve data challenges, the bloom is now off that rose and users are realizing that AI needs to be a bit more reliable, accurate and trustworthy for critical healthcare tasks.
Accurate clinical records are vital for patient care, reimbursement and operational efficiencies. When a physician creates a clinical note, they are responsible for ensuring the documentation is accurate and complete. With interoperability, we now have a wealth of additional clinical information coming from other clinicians, hospitals, labs, HIEs and other sources. While, on the surface, more information seems powerful, clinicians now need to figure out if the incoming data is accurate and what pieces are relevant for the patient in front of them.
In other words, if clinicians are responsible for the incoming tsunami of data, how can they efficiently verify and manage it all?
Consider the impact of AI-assisted documentation tools, which aim to optimize workflows, reduce documentation times and relieve physician burnout. Their output, however, is only as good as the data being fed into the systems. If a healthcare organization is building AI initiatives using data from a repository filled with errors or gaps, the resulting output will be flawed.
Transparency is thus essential when it comes to AI data. When documentation is created through AI, healthcare providers need to understand the source of truth behind an assessment or recommendation so they can quickly identify potential errors that might impact patient care.
Another limitation of conversational AI is the inability to create structured data. While these tools may be excellent at creating narrative text, they often fail to create the structured clinical data needed for analytics, regulatory compliance and quality metrics.
AI-assisted documentation tools also rely heavily on summarization, which can be beneficial when trying to quickly make sense of a compilation of dozens of multiple-page medical records. LLMs, however, are trained to identify the next logical word, with very little reasoning behind them. To drive accuracy, these models should be trained on expertly curated clinical content using advanced algorithms.
AI is also imperfect for coding, especially when derived from erroneous or incomplete documentation. Incorrect diagnosis and procedure codes can create downstream problems, including denied claims, inaccurate reimbursement and inappropriate follow-up patient care.
Healthcare enterprises using AI to generate content that becomes part of a patient's medical record should consider the following:
1. Who is responsible for the final review and approval of AI-generated content that becomes part of a medical record?
2. Where in the workflow does this happen, and how are changes or corrections made and communicated to all members of the care team?
3. Organizations should consult with clinical users to obtain agreement on how information is presented, enabling those who sign off on care plans to view documentation and, if necessary, act upon the information presented to them.
4. The organization should implement a feedback loop so users can let system and workflow designers know of issues with AI-generated content and suggest refinements and improvements.
To effectively manage today's tsunami of clinical data, especially in this era of interoperability and AI, clinicians need tools that can transform huge volumes of clinical data into a source of truth. This requires technologies that leverage trusted, curated clinical content to validate AI-generated outputs. To be useful for clinicians at the point of care, these tools should also sort and filter data and create structured, discrete information that is mapped to standard terminologies, value sets and reporting formats.
Healthcare's data tsunami is not going away. However, by embracing tools that pair with AI outputs to improve data quality, clinicians can be empowered with rapid access to accurate and actionable data that enhances patient care.

OpenAI has removed news of its deal with Jony Ive's io from its website. The takedown comes amid a trademark dispute filed by iyO, an AI hardware startup. OpenAI said it doesn't agree with the complaint and is "reviewing our options." Turns out "i" and "o" make for a popular combination of vowels in the tech industry. Sam Altman's OpenAI launched a very public partnership with io, the company owned by famed Apple designer Jony Ive, in May. The announcement included a splashy video and photos of the two of them looking like old friends. On Sunday, however, OpenAI scrubbed any mention of that partnership from its website and social media. That's because iyO, a startup spun out of Google's moonshot factory, X, and which sounds like io, is suing OpenAI, io, Altman, and Ive for trademark infringement. iyO's latest product, iyO ONE, is an "ear-worn device that uses specialized microphones and bone-conducted sound to control audio-based applications with nothing more than the user's voice," according to the suit iyO filed on June 9. The partnership between OpenAI and io, meanwhile, is rumored to be working on a similarly screen-less, voice-activated AI device. According to its deal with OpenAI, Ive's firm will lead creative direction and design at OpenAI, focusing on developing a new slate of consumer devices. When the deal was announced, neither party shared specific details about future products. However, Altman said the partnership would shape the "future of AI." iyO approached OpenAI earlier this year about a potential collaboration and funding. OpenAI declined that offer, however, and says it is now fighting the trademark lawsuit. "We don't agree with the complaint and are reviewing our options," OpenAI told Business Insider. Market Manipulation Just like the December 2024 continuing resolution, the current budget reconciliation bill fails to address the problem of Medicare reimbursing physicians at below market rates. Without a fix, the inevitable consequences will be worsening doctor shortages, declining healthcare quality, higher overall healthcare spending, and the accelerated loss of independent practices. Of course, how do we know that Medicare's payments to doctors are below their economic value? To start, Medicare's reimbursement rates have declined relative to both the cost of running a medical practice and inflation. Doctors consistently report that they lose money treating Medicare patients. A survey of doctors found that Medicare's low reimbursement rates (68%) are a primary threat to independent practices. As for inflation, it has grown 34.5% overall since January 2016 while the prices received by doctors have grown less than half as much - just 15.1%. This is not due to demand for healthcare declining – remember the huge number of aging Baby Boomers seeking health care – or a huge surge in the number of qualified doctors serving patients. In fact, the level of doctors serving patients today - 25.4 active physicians per 10,000 residents - is down from 2019 and has fallen back to 2009 levels. The problem is government setting prices at below market levels. And setting prices below market rates cause adverse consequences to our health, such as fewer doctors available to serve patients. This creates serious health risks for patients including longer wait times for appointments, less access to specialty care, shorter doctor visits, larger numbers of medical errors, and more misdiagnoses or missed diagnoses. These risks are more acute for people living in rural areas, who have a higher chance of living in areas deemed 'medical deserts' or regions that lack sufficient access to pharmacies, primary care providers, and hospitals. Medicare's uneconomical reimbursement system is also changing how patient care is delivered. For example, under the current reimbursement system, Medicare compensates hospitals more than independent physician offices for performing the same service. This incentivizes independent practices to merge with hospitals. Consolidating lower-cost physician practices into higher-cost hospital systems is driving up overall healthcare costs for patients and taxpayers and is reducing patient choice. Congress has acknowledged that there are serious consequences from government price controls, enacting legislation to raise provider payments by 2.9% for most of 2024. But these payments went back down on January 1, and the problem continues to plague the healthcare system. Fixing Medicare's flawed reimbursement policy should be a top priority for Congress. The more efficient reform comprehensively addresses Medicare's broader deficiencies by turning Medicare into a cash-based benefit system that funds health savings accounts (HSAs) for seniors. This direct payment option allows beneficiaries to receive their Medicare benefits in the same manner that they receive their Social Security benefits. At current spending levels, Medicare could give each beneficiary $15,150 annually to cover their insurance and healthcare costs. Under this system, patients and physicians, not bureaucrats, would take change and prices would reflect value. Providers would have to compete and would be incentivized to find new and better ways to expand value and reducing costs for patients. Fundamental reforms that establish well-functioning healthcare and health insurance markets will take time. Given this, Congress could act more immediately to index government payments to medical inflation and stop paying different reimbursement rates for the same services depending on where care is delivered. With more people aging, taxpayers will be spending more and more for Medicare and Medicaid. Unless Congress acts soon to stop Medicare's underpayment of physicians, it is patients who will ultimately pay the highest price with less access to doctors and specialists and longer wait times for life-saving care.
