At-Home tDCS Improves Fibromyalgia Pain

A noninvasive, home-based brain treatment with transcranial direct current stimulation (tDCS) can reduce pain severity, improve pain-related disability, and enhance endogenous pain modulation in women with fibromyalgia, suggested the results of a clinical trial published in JAMA Network Open .
The study included 112 women between the ages of 18 years and 65 years with fibromyalgia. Half received anodal tDCS (A-tDCS), which applied 2 mA of electrical current through electrodes placed on the scalp over the left dorsolateral prefrontal cortex, for 20 minutes daily and 5 days per week. The other half used a lookalike sham device that did not deliver sustained stimulation. In addition, all participants completed exercise and pain neuroscience education.
At the end of the 4-week treatment period, and after a 3-month follow-up, study participants rated their pain from 0 (none) to 10 (worst pain) for seven activities. Patients who used tDCS experienced an average 39% reduction in pain, and 62.5% of them had a 50% or more improvement in pain. In comparison, subjects who used the sham device experienced an average 16% reduction in pain. tDCS also reduced pain severity compared to sham treatment.
'These results are consistent with evidence suggesting that stimulating the left dorsolateral prefrontal cortex enhances top-down control over pain, emotion, and cognitive processing,' said study author Wolnei Caumo, MD, PhD, professor of anesthesia and pain in the medical school at Federal University of Rio Grande do Sul, Porto Alegre, Brazil. 'The moderate to large effect sizes observed reaffirm expectations about the clinical relevance of targeting the prefrontal cortex in fibromyalgia.'
How Does tDCS Reduce Fibromyalgia Pain?
Prior research suggested that tDCS applies low-level electric currents to modulate neuronal excitability and neuroplasticity. Modulating activity in the brain's dorsolateral prefrontal cortex in particular can enhance control over pain processing networks and reduce pain catastrophizing, Caumo said. 'This region is implicated in both cognitive control and emotional regulation,' said E. Baron Short , MD, MSCR, a professor of psychiatry and medicine at the Medical University of South Carolina, Charleston, South Carolina.
To understand how the placebo effect might influence the results, the patients had been randomly assigned to either treatment group and then stratified according to placebo test response; patients were evenly split between the A-tDCS and sham tDCS groups and between responders and nonresponders such that of the 56 placebo test nonresponders assigned to A-tDCS, 28 received sham treatment and 28 received active treatment, and of the 56 placebo test responders assigned to sham tDCS, 28 received sham treatment and 28 received active treatment. They found that placebo responses influenced pain intensity but not pain interference or psychosocial measures.
'While a placebo can shape subjective pain perception, functional improvements require active neuromodulation,' Caumo said. 'Moreover, placebo responders in the sham group regressed toward baseline at follow-up, while A-tDCS responders sustained their gains, highlighting the limited durability of placebo-only effects.'
In this study, tDCS didn't act alone — the education and exercise that participants completed are also beneficial in fibromyalgia.
'The evidence so far says that for chronic pain, the two therapeutic approaches that have the best evidence are pain science education and physical exercise,' said María Teresa Carrillo De La Peña, PhD, a professor of clinical psychology and psychobiology at the University of Santiago de Compostela, Santiago de Compostela, Spain, who studies novel treatments for chronic pain and cancer pain.
Exercise promotes the production of pain-relieving endorphins. And when patients learn about the brain's role in pain perception and processing, they start paying more attention to the psychological and social factors that affect their pain, she said.
'Sometimes patients think that their pain is in the knee or the back, and they are waiting for a solution for that,' she said. 'Thanks to pain science education, we stress that we need to attend to the brain.'
How Could Patients Fit Home-Based tDCS Into Their Fibromyalgia Treatment Mix?
'Given the limited and often poorly tolerated options currently available for fibromyalgia, I do believe tDCS deserves serious consideration, particularly for patients who have not responded to conventional treatments or who struggle with medication side effects,' Baron said. 'Its relative safety, ease of use, and the growing body of supportive evidence position it as a viable adjunctive treatment.'
The current study demonstrates that home-based tDCS is feasible, safe, and something patients stick with. The device used in the study was designed to be user-friendly with guidance from a healthcare professional who controls the electrode placement and timing settings.
'It reduces transportation, cost, and time barriers, [which are] especially relevant for patients with chronic pain and mobility limitations,' Caumo said. 'Additionally, the unsupervised model mimics real-world conditions and enhances scalability and autonomy, making it a cost-effective and patient-centered alternative to clinic-based sessions.'
Autonomy is powerful for patients with pain. 'One of the most important things that we are seeing right now in the management of pain is that the patient takes this active role,' Carrillo De La Peña said.

Oseltamivir Decreases Hospital Mortality Risk of Influenza

Treating patients with severe influenza with oseltamivir in the first 2 days after hospital admission was associated with a lower in-hospital mortality risk, earlier discharge, and lower readmission rate compared with supportive care, new data indicated. The additional evidence supports current guideline recommendations to treat all people hospitalized with influenza with an antiviral agent, regardless of how long they have been ill. The study, led by Anthony D. Bai, MD, assistant professor of infectious diseases at Queen's University in Kingston, Ontario, was published on June 10 in JAMA Network Open. Reduced Mortality Risk Each year, influenza causes 12,200 hospitalizations and 3500 deaths in Canada. It causes between 140,000 and 810,000 hospitalizations and 12,000 to 61,000 deaths in the United States. And despite guidelines in both countries that recommend treatment for all entering the hospital with influenza, clinical practice varies widely, Bai told Medscape Medical News . The researchers' study found that in Ontario, 30% of clinicians were not treating this population with oseltamivir. Researchers examined data from the General Medicine Inpatient Initiative database, which includes internal medicine and intensive care unit (ICU) patients admitted to participating hospitals in Ontario. Data included demographics, diagnoses, interventions, discharge, readmission, medication orders, and bloodwork results. The cohort study included 11,073 patients (average age, 72.6 years). Researchers found an adjusted risk reduction of 1.8% for in-hospital mortality when oseltamivir was given, compared with supportive care median length of stay was 4.4 days and 4.9 days in the oseltamivir and supportive care groups, respectively. After discharge, 645 patients (8.5%) in the oseltamivir group and 336 patients (9.8%) in the supportive care group were readmitted, with an adjusted risk difference of -1.5%. 'Some clinicians believe there won't be much benefit to treatment with oseltamivir,' said Bai, likely because of the dearth of evidence supporting the guidelines. 'There's been a lot of controversy about the benefit of oseltamivir in the outpatient population, where a recent meta-analysis showed it didn't really prevent hospitalizations. But there is a signal that it decreased in-hospital mortality. If a similar benefit is proven in a future randomized, controlled trial, it would add a lot of convincing evidence that there's a significant benefit to oseltamivir treatment.' The expanded version of the large RECOVERY randomized, controlled trial in the United Kingdom looks at that question, he noted. But until results are available, 'we should go by the current guideline recommendations.' Remaining Questions Wendy Sligl, MD, professor of critical care medicine and infectious diseases at the University of Alberta in Edmonton, told Medscape Medical News that the 1.8% in-hospital mortality reduction may seem small. 'Keep in mind, this is in all hospitalized patients, which includes a spectrum of disease severity from those not needing supplemental oxygen to those on a ventilator.' She pointed out that less than 10% of the patients in this study were admitted to the ICU. The number needed to treat is an important consideration with these results, she pointed out. 'Given this mortality reduction, you would need to treat approximately 55 patients to prevent one death. When the outcome is as devastating as death, and the drug is relatively safe and inexpensive, treatment seems reasonable, given even a small reduction in mortality.' Sligl also noted the need for high-quality evidence from randomized, controlled trials, including RECOVERY, in hospitalized patients with influenza. 'In addition, the most severe patients, those in the ICU with respiratory failure requiring high-flow oxygen or mechanical ventilation, should be specifically studied.' Beyond whether antiviral therapy is beneficial, she said, other questions remain, such as which antiviral is best and at what dose and duration. Other questions include whether an antiviral therapy should be used alone or in combination with another antiviral therapy and at what time in which patients. 'The best quality data we have for the treatment of influenza are in healthy outpatients, where treatment has been shown to reduce symptom severity and duration. Data for antiviral use in hospitalized patients with influenza are largely observational, suggesting a small mortality benefit with treatment,' Sligl explained. In the current study, she noted, 'target trial emulation with propensity scoring helped to minimize bias, which is a methodologic strength.' No funding source for this study was reported. Bai and Sligl reported having no relevant financial relationships.

They need trusted sources to help them navigate information overload, form informed opinions and chart a clear path forward. That's where strategic healthcare communication becomes invaluable—not by saying the most or shouting the loudest, but by communicating with clarity, calm and purpose. The most impactful messaging positions itself thoughtfully within the care continuum, defining a clear niche while acknowledging the complexity of available treatment options. A strong brand strategy doesn't just promote—it provides context. It demonstrates an understanding of the social, emotional, physical and financial barriers patients face, while staying grounded in the specific solutions you offer. Scientists Say This 10-Second Test May Predict How Long You'll Live

Our society is obsessed with longevity. Our cultural focus on working out, eating whole foods, and managing stress isn't just about improving our health now, it's about increasing lifespan and improving quality of life for years to come. And while some people turn to psychics or mediums to ask, 'When will I die?' a new study published in the European Journal of Preventive Cardiology suggests there may be a simple, science-backed test to gain insight—one you can try at home. The sitting-rising test is simple. To complete it, study participants sat on a non-slippery flat surface barefoot with clothes that didn't restrict their movements. A researcher told the participants, "Without worrying about the speed of movement, try to sit and then rise from the floor, using the minimum support that you believe is needed." Basically, people were expected to go from standing to sitting down with their legs crossed, and back up again, while using as little support as possible. For the study, researchers at the Exercise Medicine Clinic in Rio de Janeiro enlisted 4,282 adults between the ages of 46 and 75. After evaluating participants' health markers, the researchers had them perform the sitting-rising test. Each person's performance was scored from zero to five for sitting and zero to five for rising (totaling up to 10), based on how easily and independently they could complete the participants could cross their legs while sitting, they couldn't use the sides of their feet, hands, forearms, knees, or the side of their leg for support as they lowered to the ground or stood up. Each time they did, one point was subtracted from their score. Participants also lost half a point for being wobbly. If the individual completely failed at sitting or rising from the floor without external help, they were given a 0. After 12 years, researchers followed up with participants and found that 665 had died. Death rates rose sharply among those with lower sitting-rising test (SRT) scores: just 3.7 percent of people who scored a perfect 10 died, compared to 42.1 percent of those who scored between 0 and 4. They also discovered that despite other health factors, low scorers had a 3.8 times higher risk of death from natural causes and were also six times more likely to die from heart-related issues. While the study doesn't directly prove that a perfect score guarantees longevity, the authors say that health professionals can use the SRT to tailor exercise plans and gain important insights into survival prospects for middle-aged and older adults, whether healthy or with medical conditions.
