What Your Breathing Says About Your Mental Health

Welcome to Impact Factor , your weekly dose of commentary on a new medical study. I'm Dr F. Perry Wilson from the Yale School of Medicine.
You don't need to think about it until you think about it: breathing. The most fundamental physical function. The most basic act that our bodies undertake to keep our brains alive, exchanging cardon dioxide for oxygen, nourishing every cell inside us.
The reason you don't need to think about breathing is because of an area of the brainstem called the pre-Bötzinger complex. It's a group of cells that acts as a pacemaker clicking off about 12 times a minute, triggering your body, without conscious thought at all, to breathe.
Of course, you know it's more complicated than that. You can hold your breath, after all — you are in control of the process. And it's more complicated than that . Higher levels of your brain feed into the pre-Bötzinger complex to increase your breathing rate when your body is moving, or when energy expenditure goes up.
And it's even more complicated than that. You may never have noticed, but when you are breathing through your nose, you're mostly breathing through one nostril at a time, oscillating back and forth, allowing one nasal passage to regain some moisture while the other does the work.
For something you don't think about at all, breathing is taking up a lot of brain space.
Could these complex respiratory patterns reveal something, then, about the state of our brains? Are all the yogis and gurus and influencers right about the importance of how we breathe? Is the breath the window to the soul?
If you want to really interrogate the way we breathe, you need to do some pretty precise measurements — which is why I was so intrigued by this paper, appearing this week in Current Biology.
The centerpiece of the article is a new technology: a nasal cannula, but not of the sort you've seen before.
Researchers led by Timna Soroka created this device, which precisely measures the airflow in and out of each nostril. They recruited 100 people to wear it for 24 full hours.
At a sampling rate of 6 Hz, this is a ton of data. In fact, compressing that stream of data into interpretable metrics is a feat in and of itself. The primary analysis derived 24 different measures from this airflow data. Some of these are intuitive: the volume of air inhaled and exhaled, the rate of airflow, the rate of oscillation between one nostril and the other.

Some metrics are less obvious: the coefficient of variation of the breathing duty cycle, for example, appears to quantify how much one breath changes compared to others.
Our brains are unique, and, it turns out, so are our breathing patterns. The researchers wanted to know whether the 24 metrics derived from someone's breathing pattern could be used to identify them. Can breathing act like a fingerprint?
The results were impressive.
This graph shows how a computer model did when predicting which of the 100 patients a given airflow pattern came from. The diagonal lines are correct predictions. If airflow patterns were random, the machine would only get this right 1% of the time. Instead, it was correct 91% of the time. This approaches biometric levels of accuracy, similar to voice recognition.
These unique respiratory fingerprints were stable over time. Forty-two of the participants came back for another round of 24-hour monitoring, somewhere between 5 days and 2 years later. The system could identify who was who with 95% accuracy even all that time later.
This is all pretty cool, but let's be honest: You're never going to unlock your laptop by strapping a nasal cannula to your face and breathing for a while.
Where this study gets really interesting is in the links between breathing parameters and other physical and psychological parameters.
For example, the breathing pattern was associated with body mass index. People with a higher BMI had a higher tidal volume — a larger volume of air during a typical respiration — than people with a lower BMI.
That makes some sense. People with more mass might need to exchange more air to keep oxygen and carbon dioxide levels normal.
But our psychology changes how we breathe as well. The researchers divided people into low and high scorers on the Beck Depression Inventory. Now, it's worth noting that none of these participants suffered from clinical depression, but of course some had higher scores and some had lower scores. You can see here that the peak inspiratory flow, the fastest rate at which we breathe in, was substantially higher in those with more depressive symptoms.
This is not a conscious process. This is the state of your brain controlling subtle features about how you breathe that can only be revealed with new technology.
It didn't stop with depression. The researchers could distinguish between people with high vs low anxiety scores by looking at the variation in their inspiratory pause.
And you could determine who was more likely to have autistic features — again, none of these individuals had clinical autism — by looking at the percentage of breaths with an inspiratory pause.
This is where this stuff starts to get interesting, because it suggests that there are physiologic links between brain states and breathing patterns and that those links can be mapped with careful measurement.
I actually think there is a practical application to this. No, I don't think we'll all be walking around with tubes in our noses like stillsuits from Dune . But plenty of people are wearing nasal cannulas at night — for CPAP treatment. Adding technology like this to those devices could give insights into how these metrics change over time, perhaps cluing us into changes in our mood or anxiety or other health conditions before we are even consciously aware of them.
Our breathing says a lot about us. So, to paraphrase Sylvia Plath, take a deep breath and listen to the old brag of your heart. I am, I am, I am.

For 200 years, there has been speculation that swimmers could be tormented by more than one species of bluebottle jellyfish. It turns out there are at least four, including one in Australia that has a 'prominent nose'. 'We were shocked, because we assumed they were all the same species," Griffith University marine ecologist Professor Kylie Pitt said. Until recently, studying these notorious jellyfish has been a challenge because they're hard to keep in captivity, and when they wash up on the beach they disintegrate quickly. Speaking with Yahoo News, Pitt explained the game-changer has been advances in DNA sequencing. 'With our paper, we used the most powerful method, whole genome sequencing, so it's entire DNA has been sampled,' she said. 'By combining the DNA with morphology, we've got the strongest evidence possible for separating out the species.' The discovery is likely to have a direct impact on Aussie swimmers because it could help researchers better understand jellyfish behaviour and predict when they'll swarm towards beaches. Pitt was part of an international research effort by Yale University, University of New South Wales and Griffith University that sequenced the genomes of 151 jellyfish from around the world and published their findings in the journal Current Biology. A key finding of the study that the notorious Portuguese man o' war is only found in the Atlantic and is a completely separate species from Australian bluebottles. 'For a long time, people have said they're much bigger over there and the venom is much more potent,' Pitt said. 'Now we know that the potency of their venom might differ.' Related: 😳 Swimmer's painful encounter with deadly creature on Queensland island The existence of three newly described species, Physalia physalis, Physalia utriculus and Physalia megalista, had been proposed in the 18th and 19th Centuries, but the idea was later dismissed. The researchers also identified a fourth species Physalia minuta that had never been described before. 'The one that really stands out is Physalia megalista because it has a really pronounced nose,' Pitt said. It's also believed there are several distinct subpopulations shaped by regional winds and ocean currents. This new knowledge about bluebottles will have a real-world impact for swimmers, as there could be differences with the venom of Australian species too. 'Bluebottles can actually erect their float or lay it down, depending upon what the wind conditions are. And they can also expand and or extend and contract their tentacles, which might act a little bit like a sea anchor, so there might be a bit more of a drag,' she said. 'Now we know there are at least three species in Australia she may need to look at their behavioural differences.' These are just a few ways the practice of medicine (and the traditional annual physical) will change by 2050, according to Lisa Sanders, MD, professor of medicine at Yale School of Medicine and medical director of Yale's Long Covid Multidisciplinary Care Center. New COVID Variant Nimbus May Cause Painful ‘Razor Blade Throat' Symptom

The Nimbus variant of COVID-19 is on the rise, as many have reported a painful symptom they're calling 'razor blade throat.' If you have a severe sore throat, it's important to get tested to rule out strep throat to avoid severe complications. COVID-19 precautions like masking and vaccination remain important. A newly identified COVID-19 variant called 'Nimbus' is rapidly spreading in the United States and making headlines with a common symptom that's turning heads in exam rooms. People contracting a COVID-19 infection are reporting an unusually intense sore throat, described as feeling like swallowing razor blades. Some think this symptom, dubbed 'razor blade throat,' may set this variant apart from earlier ones. However, experts aren't so sure. Sore throat has long been recognized as a COVID-19 symptom. Early data suggest Nimbus may not cause more severe illness overall, but reports of severe sore throats are raising questions about how this variant might differ from its predecessors. Here's what experts told Healthline about the Nimbus variant — and what you should know. Nimbus is more transmissible, but not as severe According to the World Health Organization (WHO), Nimbus, also known as NB.1.8.1, is a subvariant of the Omicron variant of the SARS-CoV-2 virus that causes COVID-19. Yale School of Medicine Assistant Professor Scott Roberts, MD, explained that Nimbus currently accounts for about one-third of COVID-19 cases in the United States. 'It is more transmissible than the current circulating variants because of several mutations in the spike protein that make it distinct from the others,' he told Healthline. 'But, it is still an omicron subvariant related to some variants that were circulating previously, so I suspect there will be an increase in cases, but not a wave to the degree we have had previously.' Kartik Cherabuddi, MD, FIDSA, Chief Hospital Epidemiology Officer at Tampa General Hospital, noted that while Nimbus is currently surging around the world, the public health risk is still low compared to other Omicron lineages. 'Early and limited laboratory data do not show a significant capability to evade the immune system or increased resistance to antiviral treatment,' he told Healthline. Cherabuddi added that no reported studies are currently available on the impact of Nimbus on clinical outcomes. However, '[r]outine surveillance, so far, does not reveal an increase in hospitalization or death,' he said. 'Razor blade throat' may not be a new COVID symptom Roberts said the symptoms of the Nimbus variant are the same as those of prior COVID-19 variants, including fever, cough, and sore throat. 'The 'razor blade throat' is anecdotal and there is no evidence to suggest that this variant leads to more severe sore throat than other variants,' he added. Roberts advised, however, if you do have a sore throat, it would be a good idea to test for COVID-19 to make sure it is not the cause. If your sore throat is severe, he also recommends checking in with your doctor to rule out strep throat. Other common symptoms of COVID-19 include loss of taste or smell and fatigue. Less frequently, individuals may experience: sore throat headaches body aches diarrhea skin rash discoloration of fingers or toes red, irritated eyes Symptoms of strep throat may include: sudden fever sore throat with white patches headache chills loss of appetite swollen lymph nodes problems with swallowing Experts say it's important to distinguish strep throat from COVID-19. Unlike COVID-19, which is caused by a virus, strep throat is a bacterial infection caused by group A Streptococcus. Strep throat often requires antibiotic treatment to prevent potentially serious complications. These may include peritonsillar abscesses (pus-filled infections behind the tonsils), rheumatic fever (which can damage the heart, joints, and skin), post-streptococcal glomerulonephritis (a type of kidney inflammation), and, in rare cases, streptococcal toxic shock syndrome or meningitis. How to protect yourself from the Nimbus variant The first thing to know about protecting yourself from the Nimbus variant is that nothing has really changed in this department. 'The same tools that worked before will still work: handwashing, avoiding people who are sick, good ventilation, getting vaccinated, and masking in high risk settings,' said Roberts. The CDC recommends the vaccine for most adults ages 18 and older to maintain protection against severe illness, hospitalization, and death. Older adults, high risk individuals, and those who've never been vaccinated are especially encouraged to get it. Parents should consult a healthcare professional for kids under 18. The CDC additionally notes that immunity from previous vaccines wanes over time, making updated vaccination important. COVID-19 vaccine recommendations may also change soon, making it more difficult for healthy adults, children, and pregnant people to get one. Cherabuddi added there are certain symptoms that would indicate you need to consult with a physician, such as: severe throat pain throat pain that lasts longer than a week weight loss food sticking in your throat new or worsened shortness of breath 'If any of these red flags occur, have it checked out,' he said.
