ECG Challenge: On-Again, Off-Again Lightheadedness

A 72-year-old man with a history of coronary artery disease treated medically presents to his primary care provider with complaints of palpitations and occasional episodes of lightheadedness. He has had episodic lightheadedness with presyncope, but no syncope, over the past 2 months. The episodes have become more frequent over the past 2 weeks. While an ECG is obtained, he complaints of lightheadedness. Figure 1. Courtesy of Philip J. Podrid, MD. The correct diagnosis is tachycardia-bradycardia (sick sinus syndrome; Figure 2). Figure 2. Courtesy of Philip J. Podrid, MD. Discussion The initial four RR intervals are irregularly irregular. No organized P waves are seen, but there are irregular undulations of the baseline (^). The QRS complexes have a normal duration (0.08 sec) and morphology. Therefore, the initial rhythm is atrial fibrillation. The atrial fibrillation abruptly terminates after the first four QRS complexes, followed by a QRS complex (*) that has the same duration and morphology as the first four. However, no P wave occurs before this QRS complex. Therefore, this is a junctional escape complex. This is followed by three additional junctional QRS complexes (v). The RR intervals are shortening (rate faster) because the junctional complexes are from an ectopic focus which manifests a change in its rate. The last two QRS complexes have the same duration and morphology. However, they are preceded by a P wave (+) with a stable PR interval (0.16 sec). Thus, these are sinus complexes. The QT/QTc intervals are normal (320/390 msec). After the termination of the atrial fibrillation there is a delay of 8 seconds before sinus rhythm is restored. This represents a tachycardia-bradycardia (tachy-brady) syndrome, which is a common manifestation of sick sinus syndrome. Maycee Barber has hit back at Erin Blanchfield's 'unprofessional' jibe after she was forced to pull out of their UFC Fight Night main event due to a seizure UFC sensation Maycee Barber has fiercely responded to Erin Blanchfield's jibe that her life is 'a mess' following a seizure that forced her out of their bout. The 27-year-old was set to headline UFC Fight Night at the UFC Apex against Blanchfield on Saturday evening. However, in a shocking turn of events, the flyweight clash was scrapped just before their entrance due to Barber suffering a seizure backstage. ‌ Blanchfield didn't hold back in criticising her opponent after the fight was called off, branding Barber 'unprofessional' and dismissing any possibility of rescheduling their match-up. As Blanchfield's scathing remarks went viral online, it did not take long for her rival to issue a response. ‌ Barber, who had to pull out of the fight on medical grounds, stood by her professionalism. "Not what I had planned for a Sunday post. Thank you to everyone for the prayers and support," she said. "I owe an apology to UFC, [UFC matchmaker] Mick Maynard, Dana White, [UFC matchmaker] Sean Shelby and Erin Blanchfield. "P.S: My life is not messy; this is just a bad hand. We will get to the bottom of it all and be back." 'She can't control medical issues that she's having, so that's not unprofessional. I meant more the weight miss on Friday and the way she approached it,' Blanchfield said. 'I believe she was offered more time to cut weight and didn't, missed weight and then still was acting the way she was at the weigh-ins. So that's more what I meant about being unprofessional…[being unreliable] I still stand by, she pulled out of the first [fight] and now this one. 'I know she has issues, but if she does have those severe medical issues, that's why I wouldn't want to be matched up with her again,' Blanchfield continued. 'I was caught up in the moment myself, she probably does have some issues…and she probably should've pulled out sooner so that I would've had another opponent. Not being honest about her condition wasn't the right thing to do.'

AIIMS-P to use AI-powered diagnostic equipment to improve patient care

Patna: All India Institute of Medical Sciences (AIIMS), Patna, has entered into a partnership with the e-health section of the ministry of health and family welfare to develop artificial intelligence (AI) powered solutions. It has recently also procured several AI-enabled medical devices to enhance patient treatment. Head of cardiothoracic surgery department at AIIMS-Patna, Dr Sanjiv Kumar, said that with the AI revolutionising healthcare, education, and research on a large scale, it has become crucial to build a strong foundational understanding of AI principles and applications now a days. "AI algorithms can detect maladies like cancer, heart disease, and neurological problems from medical images such as X-rays, MRIs, and CT scans. AI helps doctors determine patterns that human doctors may miss, thus improving their accuracy in diagnosing patients. Even a beginner can interpret an ECG properly with the AI-driven instrument," he said. Dr Kumar further pointed out that orthopaedic department of the AIIMS-Patna has started robotic surgery for the treatment of its patients with higher accuracy. In fact, AI has revolutionised the health care system in modern times with accurate data analysis and predictive health care, he added. Recently, the state health department has decided to launch an intensive screening of TB patients by using the technology of AI. Teams from the health department would move from village to village along with hand-held X-ray machines and AI-controlled mobile kits to identify the TB-affected people. Health department sources said that at present there are nearly two lakh TB patients in the state and more than 10,000 people affected by this disease have died during the last two years (2022-23 and 2023-24). Pramod Kumar Singh, director-in-chief (disease control) of state health department, said that the proposed screening would reveal the exact number of people suffering from TB. "AI will not only help immediate detection of TB patients with the help of X-ray and data analytics but also suggest the protocol of treatment of patients on the basis of their test reports. The department hopes that the screening will be completed soon so that proper treatment of the patients may be started at the earliest," he said. Dr Satish Kumar of Nalanda Medical College and Hospital (NMCH) said that equipment based on AI technology must be procured at all the health centres of the state with a view to ensuring early diagnosis and proper treatment of the patients. These machines would facilitate in reducing unwarranted rush of patients at the hospitals by curtailing the time of treatment, he said.
