The thinking behind Rachel Reeves's spending plans for the next three years was revealed in her statement on Wednesday. It is crude and probably correct. Of the government's three priorities, there is only one that ministers can control so she will throw money at that one.
The voters care about the cost of living, immigration and the NHS. There is not much the governmen t can do about the first. It has to talk about growth and hope for the best. We are at the mercy of Donald Trump, various wars and the bond markets.
Nor do ministers think, in their heart of hearts, that there is much they can do about the Channel crossings. They have to talk about falling legal immigration, a trend they inherited from the Conservatives, while getting cross with the French for not doing enough to stop the small boats – but not so cross that the gendarmes shrug and fold their arms.
That leaves the NHS. The chancellor has put all her chips on the blue and white oblong on the casino table. The health service received the most generous settlement on Wednesday, planned to grow by three per cent more than inflation over this parliament.
There are those – and Nigel Farage is one of them – who will mutter 'bottomless pit' and 'good money after bad'. Those of us who are a bit more sophisticated will mention the NHS productivity crisis. Before the spending review, for example, I pointed to figures from the Institute for Fiscal Studies showing huge increases in the numbers of doctors and nurses in the NHS over the past five years, and small increases in the number of patients seen.
But I also cited evidence that NHS productivity was improving after the one-off shock of the pandemic and now there is more hopeful news to share. Reports are beginning to emerge about what is in the 10-year plan for the NHS to be published by Wes Streeting, the health secretary, next month. It sounds like a good and ambitious plan to shift incentives so that patients are kept out of hospitals and needless in-person appointments are abolished.
Speaking to The Times, Streeting said: 'Much of what's done in a hospital today will be done on the high street, over the phone, or through the app in a decade's time.'
It might seem a bit slow. He has been in government for nearly a year and is only now coming up with a plan?
Government is slow – Keir Starmer has taken to asking repeatedly, 'Why not today?' – but it is important to get big changes right, and Streeting has thrown himself at a lot of the less visible work in his first 11 months, including abolishing the NHS England bureaucracy and taking the NHS back under the direct control of his department.
He has learned the lessons from the last time Labour saved the NHS under Tony Blair, including bringing back some of the key people who did it: Alan Milburn, Blair's health secretary, and Michael Barber, the head of his delivery unit.
The blueprint is all there in a new book, The Art of Delivery by Michelle Clement, my colleague at King's College London. It is based on Barber's diaries and is the fullest account of how the public services were turned around in Blair's second term as prime minister.
The book makes clear what ought to be obvious, which is that it takes time for the combination of more money and reform to start to change measurable outputs, and even longer before the general public notices an improvement. Nor is improvement a steady upward gradient, because there are policy mistakes and personality clashes along the way.
One of Barber's greatest strengths was his ability to manage relationships put under strain by politicians' impatience for delivery. When one permanent secretary ranted at him for giving his department a traffic-light rating in a note to Blair without consulting him, Barber said: 'This has always happened. I'm just telling you.'
Now it is happening again. The good civil servants and NHS managers will realise that it helps them to have objective performance indicators and stretching targets if the whole service is starting to move in the right direction.
Barber had to persevere for two years before the indicators started to shift, but in the NHS the momentum of change gathered pace thereafter. By 2004, Barber told the cabinet that an episode of EastEnders showed Ian Beale complaining that 'people spend at least five hours in A&E', to which Jane, his wife, responded, 'It's a lot better nowadays.'
Barber began to talk confidently about how the changes in just three years were becoming 'irreversible' – a claim that was mocked by the parsimony and incompetence of the Tory years, which managed to reverse the Labour gains eventually.
The point is that the NHS can be changed in a single parliament. The challenges are different now, and so are the technologies. But the principles are the same: more money accompanied by devolution of power to successful managers and aligning staff incentives with the interests of patients.
Time is already running out for this government, and the stakes are high. Most cabinet ministers understand that. One of them was quoted anonymously by The Times today. If it wasn't Streeting himself, it was someone who thinks just like him: 'The truth is there are a lot of people whose lives have been shit for a long time. They rolled the dice with Brexit, they rolled the dice with Boris and then they rolled the dice again with us. They need to see results otherwise they will roll the dice again with Reform.'
Time is running out, but Streeting is one of the few cabinet ministers to have made good use of it so far.

Vladimir Putin has said he believes the whole of Ukraine is 'ours' and warned that advancing Russian forces could take the northern Ukrainian city of Sumy as part of a bid to carve out a buffer zone along the border. Ukraine's foreign minister denounced the Russian president's statements on Friday as evidence of Russian 'disdain' for US peace efforts and said Moscow was bent on seizing more territory and killing more Ukrainians. Putin, when asked about fresh Russian advances, told the country's flagship economic forum in St Petersburg that he considered Russians and Ukrainians to be one people and 'in that sense the whole of Ukraine is ours'. Volodymyr Zelenskyy has repeatedly rejected the notion that Russians and Ukrainians are one people. The Ukrainian president said commanders had discussed Russian action in the Sumy region and 'we are holding them back and eliminating these killers'. Vladimir Putin claimed he was not questioning Ukraine's independence or its people's striving for sovereignty but that when Ukraine declared independence as the Soviet Union fell in 1991 it also declared its neutrality. He said Moscow wanted Ukraine to accept the reality on the ground – where Russia now controls about a fifth of Ukraine – if there was to be a chance of peace. Andrii Sybiha, the Ukrainian foreign minister, said on X: 'Putin's cynical statements demonstrate complete disdain for US peace efforts. While the United States and the rest of the world have called for an immediate end to the killing, Russia's top war criminal discusses plans to seize more Ukrainian territory and kill more Ukrainians.' Zelenskyy said in his nightly video address that Russia had shown 'openly and utterly cynically that they 'don't feel like' agreeing to a ceasefire. Russia wants to continue the war.' Russia and Ukraine exchanged more captured soldiers on Friday, the latest in a series of prisoner swaps agreed at peace talks in Istanbul earlier this month. 'A group of Russian servicemen was returned from the territory controlled by the Kyiv regime,' Russia's defence ministry said. 'In exchange, a group of Ukrainian prisoners of war was handed over.' An Agence France-Presse reporter saw freed Ukrainian prisoners of war being greeted by tearful relatives after stepping off a bus. Zelenskyy said most of the Ukrainians freed in the swap had been in Russian captivity 'for over two years'. Moscow posted a video of Russian soldiers in military fatigues chanting 'Russia, Russia' with Russian flags draped over them. Neither side said how many soldiers were freed in Friday's swap. Volodymyr Zelenskyy said Ukraine was working on the rapid development of interceptor drones to counter the swarms of Russian drones that have been descending on Ukrainian cities in increasing numbers in recent weeks. 'Several of our domestic enterprises – and, accordingly, different types of drones – are delivering results,' he said. 'Production volumes of interceptors are already increasing.' Adam Greenfield was home nursing a cold when his girlfriend raced in to tell him Immigration and Customs Enforcement vehicles were pulling up in their trendy San Diego neighborhood. The poet and podcast producer grabbed his iPhone and bolted out the door barefoot, joining a handful of neighbors recording masked agents raiding a popular Italian restaurant nearby, as they yelled at the officers to leave. An hour later, the crowd had grown to nearly 75 people, with many in front of the agents' vehicles. 'I couldn't stay silent,' Greenfield said. 'It was literally outside of my front door.' More Americans are witnessing people being hauled off as they shop, exercise at the gym, dine out and otherwise go about their daily lives as President Donald Trump 's administration aggressively works to increase immigration arrests. As the raids touch the lives of people who aren't immigrants themselves, many Americans who rarely, if ever, participated in civil disobedience are rushing out to record the actions on their phones and launch impromptu protests. Arrests are being made outside gyms, busy restaurants Greenfield said on the evening of the May 30 raid, the crowd included grandparents, retired military members, hippies, and restaurant patrons arriving for date night. Authorities threw flash bangs to force the crowd back and then drove off with four detained workers, he said. 'To do this, at 5 o'clock, right at the dinner rush, right on a busy intersection with multiple restaurants, they were trying to make a statement,' Greenfield said. "But I don't know if their intended point is getting across the way they want it to. I think it is sparking more backlash.' Previously many arrests happened late at night or in the pre-dawn hours by agents waiting outside people's homes as they left for work or outside their work sites when they finished their day. When ICE raided another popular restaurant in San Diego in 2008, agents did it in the early morning without incident. White House border czar Tom Homan has said agents are being forced to do more arrests in communities because of sanctuary policies that limit cooperation with ICE in certain cities and states. ICE enforces immigration laws nationwide but seeks state and local help in alerting federal authorities of immigrants wanted for deportation and holding that person until federal officers take custody. Vice President JD Vance during a visit to Los Angeles on Friday said those policies have given agents 'a bit of a morale problem because they've had the local government in this community tell them that they're not allowed to do their job." 'When that Border Patrol agent goes out to do their job, they said within 15 minutes they have protesters, sometimes violent protesters who are in their face obstructing them,' he said. 'It was like a scene out of a movie' Melyssa Rivas had just arrived at her office in the Los Angeles suburb of Downey, California one morning last week when she heard the frightened screams of young women. She went outside to find the women confronting nearly a dozen masked federal agents who had surrounded a man kneeling on the pavement. 'It was like a scene out of a movie,' Rivas said. 'They all had their faces covered and were standing over this man who was clearly traumatized. And there are these young girls screaming at the top of their lungs.' As Rivas began recording the interaction, a growing group of neighbors shouted at the agents to leave the man alone. They eventually drove off in vehicles, without detaining him, video shows. Rivas spoke to the man afterward, who told her the agents had arrived at the car wash where he worked that morning, then pursued him as he fled on his bicycle. It was one of several recent workplace raids in the majority-Latino city. The same day, federal agents were seen at a Home Depot, a construction site and an LA Fitness gym. It wasn't immediately clear how many people had been detained. 'Everyone is just rattled,' said Alex Frayde, an employee at LA Fitness who said he saw the agents outside the gym and stood at the entrance, ready to turn them away as another employee warned customers about the sighting. In the end, the agents never came in. Communities protest around ICE buildings Arrests at immigration courts and other ICE buildings have also prompted emotional scenes as masked agents have turned up to detain people going to routine appointments and hearings. In the city of Spokane in rural eastern Washington state, hundreds of people rushed to protest outside an ICE building June 11 after former city councilor Ben Stuckart posted on Facebook. Stuckart wrote that he was a legal guardian of a Venezuelan asylum seeker who who went to check in at the ICE building only to be detained. His Venezuelan roommate was also detained. Both men had permission to live and work in the U.S. temporarily under humanitarian parole, Stuckart told The Associated Press. 'I am going to sit in front of the bus,' Stuckart wrote, referring to the van that was set to transport the two men to an ICE detention center in Tacoma. 'The Latino community needs the rest of our community now. Not tonight, not Saturday but right now!!!!' The city of roughly 230,000 is the seat of Spokane County, where just over half of voters cast ballots for Trump in the 2024 presidential election. Stuckart was touched to see his mother's caregiver among the demonstrators. 'She was just like, 'I'm here because I love your mom, and I love you, and if you or your friends need help, then I want to help,'' he said through tears. By evening, the Spokane Police Department sent over 180 officers, with some using pepper balls, to disperse protesters. Over 30 people were arrested, including Stuckart who blocked the transport van with others. He was later released. Aysha Mercer, a stay-at-home mother of three, said she is 'not political in any way, shape or form." But many children in her Spokane neighborhood -- who play in her yard and jump on her trampoline -- come from immigrant families, and the thought of them being affected by deportations was 'unacceptable," she said. She said she wasn't able to go to Stuckart's protest. But she marched for the first time in her life on June 14, joining millions in 'No Kings' protests across the country. 'I don't think I've ever felt as strongly as I do right this here second,' she said. _____
